摘 要:目的探讨小儿年龄与加强型气管插管内径的相关性。方法择期在全身麻醉下行手术的患儿40例,年龄2个月~7岁,体重7~25kg,ASA分级Ⅰ或Ⅱ级。麻醉诱导后,用无线视频喉镜暴露声门,评估声门的大小并结合传统计算公式"ID=4+(1/4)×年龄"小一号来选择相应的加强型气管插管。气管插管后连接呼吸机行机械通气。记录该患儿年龄与最终选择的导管内径值,分析二者的相关性,并记录依据传统公式计算出来的型号插管成功的总次数。结果40例患儿中,有15例能顺利通过声门,插管成功率为37.5%。40例患儿所插加强型气管插管的内径与年龄呈正相关关系(r=0.960,P<0.01),回归方程为Y=2.455+0.395X,因此加强型气管插管内径可参考回归方程简化为ID=2.5+0.4×年龄。结论在小儿全身麻醉中,年龄与加强型气管插管内径呈正相关,公式"ID=2.5+0.4×年龄"更适合小儿加强型气管插管的选择。Objective To evaluate the correlation between children’s age and internal diameter of strengthen endotracheal tube. Methods 40 ASA physical statusⅠorⅡ,aged 2m^7yr,weight 7~25kg of children, were enrolled for elective surgery with general anesthesia. After induction of anesthesia,the glottis was exposed with wireless video laryngoscope,the glottal aperture was e-valuated and the corresponding endotracheal tube was selected combined with smaller than traditional models according to the formula“ID=4(1/4) × age”. After tracheal intubation,tracheal connected to a ventilator for mechanical ventilation. The values of tube di-ameter and children’s age were recorded,the correlation between these values was analyzed,and the total number of successful intuba-tion was recorded based on the model of traditional formula calculated. Results In 40 cases,15 cases could go smoothly through the glottis,the success rate of intubation was 37. 5%. The correlation analysis between the children’s age and internal diameter of strengthen endotracheal tube showed highly positive correlation(r=0. 960a,P〈0. 01). The regression equation was Y=2. 455+0. 395X. Therefore,the tube diameter can be reference regression equation and the formula was simplified to ID=2. 5+0. 4 × year. Conclusion In children’s general anesthesia,the age and internal diameter of strengthen endotracheal tube showed highly positive correlation,the formula“ID=2. 5+0. 4 × year” is more suitable for the selection of strengthen endotracheal tube in children.
