缘对缘瓣叶缝合技术治疗退行性二尖瓣关闭不全的近、远期效果研究

摘　　要：目的 研究缘对缘瓣叶缝合技术治疗退行性二尖瓣关闭不全的近远期疗效.方法 抽取2008年1月至2011年1月收治的60例退行性二尖辩关闭不全患者为研究对象,均行缘对缘瓣叶缝合技术治疗,其中同期接受瓣环成形术45例,观察和记录手术结果,术后随访1 ～3（2.0±0.5）年,观察和比较手术前后患者超声心动图指标、二尖瓣反流改善及随访结果.结果 手术前后超声心动图检查,术后左心室射血分数、左心室舒张/收缩末期内径、左心房容积、二尖瓣反流分级较术前均明显改善,差异有统计学意义（P＜0.05）.术后随访1～3年,死亡2例;二尖瓣重度反流1例,中度反流2例,无狭窄发生.结论 缘对缘瓣叶缝合技术治疗退行性二尖瓣关闭不全近期疗效明确,安全性高,联合瓣环缝合术能有效提高远期效果.Objective To study the short-term and long-term curative effects of edge to edge flap leaves suture technique on degenerative mitral insufficiency.Methods Sixty patients with degenerative mitral insufficiency from January 2008 to January 2011 were selected as the research objects,all with edge to edge flap suture technique,which at the same time accept the disc ring keratoplasty 45 cases,the results were observed and recorded,postoperative follow-up was 1-3 （2.0 ± 0.5） years,echocardiography index,mitral valve regurgitation improvement and follow-up results before and after surgery were compared.Results Echocardiography before and after surgery,postoperative left ventricular ejection fraction,left ventricular diastolic/end systolic diameter,left atrial olume index,grade of mitral regurgitation in the preoperative were significantly improved,the differences were significant （P 〈 0.05）.Postoperative follow-up of 1-3 years,2 cases died;1 case of mitral valve severe reflux,moderate reflux in 2 cases,no mitral stenosis occured.Conclusions The shortterm curative effect of edge of velar leaf suture technique on degenerative mitral insufficiency is good,has high safety,joint disc ring suture technique can effectively improve the long-term outcome.
