粗隆间假体在高龄不稳定型转子间骨折的应用

摘　　要：[目的]探索粗隆间假体在高龄患者转子间各型骨折中的应用及疗效评估。[方法]2006年5月~2014年9月采用粗隆间假体行半髋置换术治疗高龄患者转子间不稳定骨折37例。[结果]37例中31例获6个月~8年的随访,术后6个月,31例患者均达到满意的功能恢复,Harris评分78~97分,平均为91分;1例出现假体周围骨折,行钢板加钢丝固定术;未出现骨溶解、假体松动;未发现感染病例。[结论]采用粗隆间假体行单髋置换术适用于各种类型高龄患者转子间不稳定骨折的治疗,临床疗效满意。[ Objective] To investigate and evaluated the clinical outcomes of a specially designed semi -arthroplasty pros- thesis treating unstable intertrochantefic fractures in the elderly patients with osteoporosis. [ Method ] From May 2006 to Sept 2014,we performed hemiarthroplasty in 37 elderly patients with osteoporotic unstable intertrochanteric fracture using a specially designed semi - arthroplasty prostheses. [ Result] Of the 37 patients ,31 cases were followed up from 0.5 year to 8 years. All of them recovered for satisfactory function in daily life. The Harris score at 6 month postoperatively was 91 in an average,ranged from 78 to 97. During the follow - up period, one hip was refixed with plate and steal wire due to fractures around the prosthesis of hip. However, no complications such as prosthesis loosening and osteolysis happened in any remaining cases of unstable inter- trochanteric fractures treated with the new prosthetic system. [ Conclusion ] For treatment of unstable intertrochanteric fractures in the elderly patients with osteoporosis, hemiarthroplasty using specially designed semi - arthroplasty prosthesis could lead to satisfactory outcomes.
