3种内膜准备方案在多囊卵巢综合征患者冻融胚胎移植中的应用比较

摘　　要：目的比较多囊卵巢综合征(PCOS)患者冻融胚胎移植周期(FET)3种不同方案准备后临床诊疗结局,探讨该类患者适合的FET助孕方案。方法回顾性分析助孕的PCOS患者的146个FET周期临床资料,根据其FET方案将患者分为自然周期组(NC)19个、激素补充组(HRT)92个、促排周期组(COH)35个,比较3组一般资料及胚胎种植率、临床妊娠率、生化妊娠率、早期流产率等助孕结局。结果 3组比较,患者一般资料如年龄、不孕年限、体质量指数(BMI)、ET日内膜厚度及胚胎复苏情况差异无统计学意义,3组间及2组间比较生化妊娠率差异均无统计学意义(P>0.05);临床妊娠率3组间比较COH组(74.3%)高于NC组(36.8%)及HRT组(43.5%),差异有统计学意义(P<0.05);临床妊娠率2组间比较结果,NC组(36.8%)与HRT组(43.5%)差异无统计学意义(P>0.05);COH组显著高于NC组(36.8%)及HRT组(43.5%,P<0.05)。种植率3组间比较,COH组(44.3%)高于NC组(17.1%)及HRT组(29.4%),差异无统计学意义(P>0.05);种植率2组间比较,COH组(44.3%)高于NC组(17.1%,P<0.05),其余比较差异无统计学意义(P>0.05)。早期流产率3组间及2组间比较差异均无统计学意义(P>0.05)。结论 PCOS患者FET助孕周期3种内膜准备方案COH能获得更好的临床妊娠率,但临床工作中应选择个体化用药方案。Objective To compare the clinical outcomes of the three different endometrial preparation protocols for frozen-thawed embryo transfer（FET） cycle in patients with polycystic ovary syndrome（PCOS）. Methods Three clinical protocols for preparing endometrium were carried out including natural cycle group（NC group）,hormone replacement group（HRT group） and ovulation induction group（COH group）. The rates of biochemical pregnancy,clinical pregnancy,embryo implantation,early abortion,and the patients age,BMI,endometrial thickness were compared among these protocols. Results No significant differences were found in patients age,BMI,endometrial thickness,the rates of biochemical pregnancy,clinical pregnancy,embryo implantation,early abortion among the three groups（P〉0. 05）. The clinical pregnancy rate of COH group was significantly higher than the other two groups（P〈0. 05）.Conclusion Thisstudy suggests that all of the three endometrial preparation protocols can achieve a good pregnancy outcome on FET cycle for patients with PCOS among three protocols,COH protocol is the best choice.
