摘 要:目的查找国内外手传振动对作业人员心血管系统影响的研究结果,进行Meta分析,以期得到更有说服力的结论,为预防和诊断提供基础资料。方法检索和收集截止2013年6月国内外发表的关于职业性手传振动对作业人员心血管系统影响的文献,经筛选后,应用RevMan 5.2软件对数据进行合并,采用Meta分析合并相对危险度(RR)值作为各项目的总效应指标。结果共纳入51篇文献,与对照组相比,职业性手传振动接触作业人员手臂振动病(RR=15.57,95%CI:4.79~50.59)、振动性白指(VWF,RR=2.89,95%CI:2.63~3.16)、高血压(RR=2.92,95%CI:1.95~4.37)及心电图ST-T改变(RR=4.02,95%CI:1.33~12.11)、传导阻滞(RR=4.00,95%CI:1.82~8.78)、窦性心动过缓(RR=2.98,95%CI:2.03~4.39)、窦性心律不齐(RR=1.92,95%CI:1.34~2.74)的发生与对照组的差异有统计学意义(P〈0.05)。结论长期接触职业性手传振动可引起手臂振动病、VWF、高血压及心电图ST-T改变、传导阻滞、窦性心动过缓、窦性心律不齐的发生。Objective To analyze and evaluate the effects of occupational hand-transmitted vibration exposure to workers using a Meta-analysis.Methods We systematically searched cross-sectional and cohort studies about the effects of occupational hand-transmitted vibration on cardiovascular system at home and abroad which were made before June 2013.Then all the literatures were reviewed in according to inclusion and exclusion criteria.RevMan 5.2software was used to merger and analyze data from each included study.Results In total,51 literatures were included.Compared with the controls,the occupational exposure to hand-transmitted vibration was significantly associated with the occurrence of hand-arm vibration syndrome(RR=15.57,95%CI:4.79~50.59),vibration-induced white finger(RR =2.89,95%CI:2.63~3.16),hypertension(RR =2.92,95%CI:1.95~4.37),ECG ST-T change(RR =4.02,95%CI:1.33~12.11),ECG conduction block(RR=4.00,95%CI:1.82~8.78),ECG sinus bradycardia(RR=2.98,95%CI:2.03~4.39),ECG sinus arrhythmia(RR =1.92,95%CI:1.34~2.74)(P〈0.05).Conclusions Occupational exposure to hand-transmitted vibration can cause hand-arm vibration syndrome,vibration-induced white finger,hypertension,ECG ST-T change,ECG conduction block,ECG sinus bradycardia and ECG sinus arrhythmia.
