摘 要:目的了解石油化工(简称石化)作业对工人肺通气功能的影响,为更好地保护石化作业工人健康提供科学依据。方法采用整群抽样方法,通过问卷调查选择无吸烟史且半年内无呼吸系统疾病史的兰州市某石化企业接触刺激性气体工人(369人)和接触噪声工人(377人)分别为暴露组和对照组,采用spirovit SP-1型肺功能仪进行肺通气功能检查,检查指标为FVC、FEVl和FEVl/FVC。结果与对照组比较,暴露组FEVl、FEVl/FVC分别降低5.3%、4.8%(P<0.05);暴露组肺功能异常率、阻塞性肺功能损伤发生率分别增高10.7%、12.9%(P<0.05),其中,以男性、工龄≥20a者和年龄≥40岁者较高(P<0.05)。结论石化作业对工人肺通气功能有一定危害,主要表现为阻塞性肺通气功能障碍,男性、工龄≥20a者和年龄≥40岁者损害明显,应定期对石化作业工人肺功能进行检查。Objective To investigate the effects of workers exposed in petrochemical industry in order to provide scientific evidence for protection of petrochemical workers' health.Methods Through cluster sampling method,two groups of workers having no smoking history and respiration system disease within six month were selected for the survey.One group exposed to the irritant gas was referred as exposure group and another group exposed to noise as control group.Spirovit SP-1 spirometer was applied to determine respiration physiological parameters:FVC,FEV1 and FEV1/FVC.Results Compared to the control group,the exposure group had lower FEV1 and FEV1/FVC values by 5.3% and 4.8%(P〈0.05),and higher incidence of pulmonary function abnormality by 10.7%(P〈0.05)and obstructive pulmonary function injury more than 12.9%(P〈0.05).High incidences were found among the male workers,those aged older than 40 years and having at least 20 years length of service.Conclusions Petrochemical operation could seriously harm to workers' pulmonary ventilation function with respect to abnormal pulmonary function and obstructive pulmonary function disorders,especially,among the male workers,those older than 40 years and having the at least 20 years length of service.It is recommended that regular examination of pulmonary function should be implemented.
