摘  要:目的探讨盆底器宫脱垂患者子宫骶韧带8一羟基脱氧鸟苷(8-hydroxy-2-deoxyguanosine,8-OHdG)及4-羟基壬烯醛(4-hydroxynonenal,4-HNE)的表达情况,并分析其与盆底器官脱垂(pelvicorganprolapsed,POP)的发病关系。方法选取2012年6月至2014年6月在武汉大学人民医院59例行子宫全切术患者资料,分为Non—POP组20例(对照组)、轻度POP组17例(POPI—Ⅱ)、重度POP组22例(POPⅢ~Ⅳ)3组,于术中切取患者部分子宫骶韧带组织,通过免疫组化的方法检测8-OHdG和4-HNE的表达情况,比较其在各组的差异并作相关性分析。结果8-OHdG和4-HNE在POP组中的表达高于对照组,其中重度POP组最高,各组比较差异有统计学意义(P〈0.05),并且两者在POP组表达呈正相关(P〈0.05)。结论氧化损伤随脱垂程度的加重而增加,子宫骶韧带组织细胞的氧化损伤可能是POP的发病机制之一。Objective To investigate expression of 8 - hydroxy - 2 - deoxyguanosine (8 - OHdG) and (4 - hydroxynonenal, 4 - HNE) in uterosacral ligament of women with pelvic organ prolapsed, and speculate the relationship between oxidative damage and POP. Methods The clinical data of 59 cases underwent total hysterectomy in People' s Hospital of Wuhan University form Jan 2012 to Jan 2014 were selected, all cases were divided into 3 groups: control group (20 cases), POP Ⅰ -Ⅱ group (17 cases), POPⅢ - Ⅳ group ( 22 eases), specimen of uterosacral were taken from patients, immunohistochemistry was used to detect 8 - OHdG and 4 - HNE, differences were compared in 3 groups and the correlations were analyzed. Results The level of 8 - OHdG and 4 - HNE in POP groups were higher than in control group, the POPⅢ - Ⅳ group was the highest, the differences had statistical significance among the 3 groups ( P 〈 0.05 ), the expressions of 8 - OHdG and 4 - HNE were positively correlated in POP groups ( P 〈 0. 05 ). Conclusion Oxidative damage increases with the severity of prolapse, and oxidative damage in uterosacral ligament may play an important role in pathogenesis of POP.
