电针刺激对手术中肺萎陷—复张过程中炎症因子变化的影响

摘　　要：目的胸腔手术时会在肺的萎陷—复张过程中产生一系列炎症反应,通过肺内炎症因子的测定,验证电针刺激能够通过调节炎症因子起到减轻肺部炎症的作用。方法取开胸手术患者40例,随机平均分为电针手术组和常规手术组,在肺萎陷和肺复张两个时间点抽取肺静脉血,测定炎症因子[白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、肿瘤坏死因子(TNF)]并加以比较。结果 IL-2、IL-10、TNF含量在肺萎陷—复张过程中无显著变化;IL-6含量在电针手术组中有显著增高(P<0.01),而在常规手术组中增高不明显(P>0.05),两组之间存在差异(P<0.05);IL-8含量在电针手术组中显著降低(P<0.05),常规手术组虽同样下降,但下降差异不显著(P>0.05),两组间不存在差异(P>0.05)。结论电针刺激确实引起了部分炎症因子(IL-6、IL-8)的变化,但其对肺内炎症改变的影响和调控过程有待于进一步研究阐明。Objective A series of inflammatory reaction occurs during lung collapse - re - expan-sion during thoracic surgery. Through determining pulmonary inflammatory factors,the research was to verify the effect of electroacupuncture stimulation（EA）for alleviating pulmonary inflammation via regulating inflam-matory factor. Methods Forty patients with chest operation were randomized evenly into an EA operation group and a conventional operation group. The pulmonary venous blood was collected at two time points, named lung collapse and lung re - expansion. The levels of IL - 2,IL - 6,IL - 8,IL - 10 and TNF were de-termined and compared. Results The levels of IL - 2,IL - 10 and TNF did not change significantly during lung collapse - re - expansion process. IL - 6 level was increased significantly in the EA operation group（P﹤ 0. 01）,and the increase of it was not apparent in the conventional operation group（P ﹥ 0. 05）. The differ-ence was significant between the two groups（P ﹤ 0. 05）. The level of IL - 8 was reduced significantly in the EA operation（P ﹤ 0. 05）and was reduced in the conventional operation group,but the change was not signifi-cant（P ﹥ 0. 05）. The difference in the level of IL - 8 was not significant between the two groups（P ﹥ 0. 05）.Conclusion EA stimulation definitely induces the changes in a part of inflammatory factors（IL - 6 and IL -8）. It is expected to have a further study of EA stimulation on the impacts and regulation of the pulmonary in-flammatory symptoms.
