用药指导在重度卵巢型内异症术后辅助治疗中的临床观察

摘　　要：目的探讨重度卵巢型子宫内膜异位症(OEM)手术后辅助促性腺激素释放激素激动剂(Gn RH-a)干预的临床疗效。方法回顾性研究2011年1月至2013年6月在我院行手术治疗的的100例重度OME患者的临床资料。按照术后是否辅助药物治疗分为两组,用药指导组50例,未用药指导组50例。所有患者随访2年,观察术后缓解情况、复发率、有生育要求患者的妊娠情况以及用药不良反应。结果1用药指导组及未用药指导组缓解率分别为92%、74.0%,两用药指导组缓解率明显高于未用药指导组,差异具有统计学意义(χ2=4.723,P<0.05);随访2年,用药指导组及未用药指导组术后复发率分别为6%、30%,用药指导组明显低于未用药指导组,差异具有统计学意义(χ2=4.982,P<0.05);2随访2年,术前有生育意愿患者共49例,术后自然妊娠率,用药指导组及未用药指导组的自然妊娠率分别为83.33%(20/24)、44.00%(11/25);用药指导组显著高于未用药指导组,两组比较差异有统计学意义(χ2=3.271,P<0.05)。结论重度OME手术后辅助Gn RH-a治疗可提高治疗效果,降低术后复发率,且不良反应少,提高自然妊娠率。Objective To investigate the clinical effect of the treatment of severe ovarian endometriosis（OEM） and the clinical effect of the treatment of the patients with Gn RH-a. Methods The clinical data of 100 patients with severe OME who underwent surgical treatment in our hospital from January 2011 to June 2013 were retrospectively studied. According to the postoperative adjuvant drug treatment is divided into two groups, 50 cases in the treatment group, 50 cases in the guidance group. All patients were followed up for 2 years, and the patients were followed up for years. The patients were followed up, the recurrence rate, the pregnancy status and the adverse drug reactions were observed. Results（1） The remission rate was significantly higher than that in the non medication group（χ2=4.723, P〈0.05）, and the difference was statistically significant（χ2=4.982, P〈0.05） in 2 year, 92% year, 74% year, 6% year and 30% year;（2） 2 years of follow-up, there were 49 patients with fertility intention, the natural pregnancy rate, 83.33%（20/24）, 44%（11/25）, the difference was statistically significant（P〈0.05, χ2=3.271）. Conclusion Gn RH-a can improve the treatment effect, reduce the recurrence rate and improve the natural pregnancy rate after severe OME operation.
