皮层-咽部-舌根序贯针刺法治疗脑出血后吞咽障碍62例临床观察

摘　　要：目的观察"皮层-咽部-舌根"序贯针刺法治疗脑出血后吞咽障碍的临床疗效。方法将62例脑出血患者随机分为治疗组和对照组,治疗组接受"皮层-咽部-舌根"序贯针刺治疗,即分别在头皮、咽部及舌根进行针刺治疗。两组均接受基础治疗及吞咽训练治疗,1次/d,30 min/次,连续治疗21 d,应用洼田饮水试验评分进行评价。结果在治疗21 d后进行统计学分析,对照组治疗前后比较,差异有统计学意义(P<0.05),治疗组治疗前后比较,差异有统计学意义(P<0.01),两组治疗后组间比较,差异有统计学意义(P<0.05);在总有效率方面,治疗组与对照组比较,差异有统计学意义(P<0.05),愈显率方面比较,差异有统计学意义(P<0.01);说明改善吞咽障碍敏感性方面治疗组优于对照组。结论在传统的吞咽训练联合药物治疗的基础上,加用"皮层-咽部-舌根"序贯针刺法能够更好地改善脑出血后吞咽障碍。Objective To observe the clinical efficacy on post- cerebral hemorrhagic dysphasia treated with cortex - throat - tongue root sequence acupuncture therapy. Methods Sixty - two cases of cere- bral hemorrhage were randomized into a treatment group and a control group. In the treatment group, the cor- tex- throat- tongue root sequence acupuncture therapy was given, in which acupuncture was applied to the cortex, throat and tongue root separately. The basic treatment and swallowing training were all applied in the two groups, once a day,30min each time, continuously for 21 days. The water - swallowing test was used for the evaluation. Results The statistical analysis was done in 21 days of treatment. The differences were sig- nificant before and after treatment in the control group （ P 〈 0.05 ）. The differences were significant before and after treatment in the treatment group （ P 〈 0.05 ）. The differences were significant between the two groups after treatment（P 〈 0.05 ）. The difference in the total effective rate was significant between the treat- ment group and the control group （ P 〈 0.05 ）. The difference in the curative and remarkably effective rate was significant between the two groups（ P 〈 0.01 ）. It was explained that the treatment in the treatment group was advantageous in the improvement of sensitivity for dysphagia as compared with the control group. Conclusion On the basis of the traditional swallowing training in combination with medication, the cortex - throat - tongue root sequence acupuncture therapy better improves in dysphagia after cerebral hemorrhage.
