摘 要:目的探讨先天性脊柱侧凸患者中限制性通气功能障碍与体重指数降低的关系,并确定低体重指数相关的因素。方法回顾性分析88例胸段先天性脊柱侧凸患者,患者均进行了肺功能测试,脊柱X线片测量,计算年龄修正的体重指数(body mass intex,BMI)及BMI百分位数,限制性肺功能障碍的严重程度采用用力肺活量(forced vital capacity,FVC)百分率量化。BMI百分位数分为正常、轻度降低(小于1个标准差)、显著降低(小于2个标准差);统计学采用t检验验证BMI分类和FVC%和一秒用力呼气容积(forced expiratory volume in one second,FEV1)间的相关性,多元线性回归用于BMI百分数、FVC%、FEV1%、年龄、性别、脊柱侧凸类型、放射学测量和肺功能测量的分析。结果所有患者的平均BMI百分数为38.0%(0%~99%),FVC%和FEV1%数都普遍降低。统计分析显示FVC%和FEV1%与BMI百分位数降低具有显著相关性。结论先天性脊柱侧凸患者中,肺功能降低和低BMI有关系,具有限制性肺功能障碍的患者更易有BMI减低的风险。Objective To investigate the relationship between the restrictive ventilation dysfunction and the body mass index in patients with congenital scoliosis, and to determine the factors associated with low body mass index. Method 88 cases of thoracic congenital spinal scoliosis patients were retrospectively analyzed. All patients had been taken pulmonary function test;spinal X-ray measurement;and age adjusted body mass index ( BMI) and BMI per-centile digit were calculated. The severity of limit pulmonary dysfunction was quantified by percentage of force vital capacity. BMI percentile was pointed as normal, mildly reduced (less than 1 standard deviation), and decreased significantly ( less than 2 standard difference) . The correlation between BMI classification and FVC% and one sec-ond forced expiratory volume ( FEV1 ) were verified by t test. The percentage of FVC%, FEV1%, BMI, age, sex, type of scoliosis, radiological measurement, and pulmonary function were analyzed by multivariate linear regression. Result The average BMI percentage of all patients was 38% ( 0% to 99%) , and the number of FVC% and FEV1% was decreased. Statistical analysis showed that FEV1% and FVC% had a significant correlation with the decrease of BMI percentile. Conclusion In patients with congenital scoliosis, pulmonary function and low BMI are related, and patients with restricted pulmonary function are more likely to have the risk of BMI reduction.
