超声弹性成像与彩色多普勒超声成像对乳腺实性肿块的诊断价值

摘　　要：目的：探讨超声弹性成像与彩色多普勒超声成像对乳腺实性肿块的鉴别诊断价值。方法：对2010年3月～2015年3月收治的300例乳腺实性肿块患者进行回顾性分析，以病理诊断为标准，将这些患者分为超声弹性成像组、彩色多普勒超声成像组和联合诊断纽各100例，对比分析3组检测方法对乳腺实性肿块诊断的敏感性、特异性和准确性。结果：超声弹性成像组的敏感性、特异性和准确性分别为90．4％，92．5％和95．6％，与彩色多普勒超声成像组的敏感性、特异性和准确性（80．4％，82．5％和85．6％）之间存在显著差异，具有统计学意义（P〈0．05）；联合诊断组的敏感性、特异性和准确性分别为92．4％，94．5％和97．6％，与彩色多普勒超声成像组的敏感性、特异性和准确性之间存在显著差异，具有统计学意义（p〈0．05）；超声弹性成像组的敏感性、特异性和准确性与联合诊断组之间没有显著差异，无统计学意义（P＞0．05）。结论：采用超声弹性成像可以更准确地诊断乳腺实性肿块情况。诊断结果和超声弹性成像与彩色多普勒超声成像联合的诊断结果基本一致。具有重要的临床意义。Objective To study the differential diagnosis value of ultrasound elasticity imaging and color Doppler ultrasound imaging for the breast solid tumor. Methods From March 2010 to March 2015, 300 patients with breast solid tumors in our hospital were retrospectively analyzed. Based on the pathological diagnosis, patients were divided into ultrasound elasticity imaging group, color Doppler ultrasound imaging group and combined diagnosis group, each group of 100 patients. The sensitivity, specificity and accuracy of these three groups for the breast solid tumor were compared and analyzed. Results The sensitivity, specificity and accuracy of ultrasound elasticity imaging group were respectively 90.4%, 92.5% and 95.6%, while those of color Doppler ultrasound imaging group were respectively 80.4%, 82.5% and 85.6%, with statistical significance （P〈 0.05）. The sensitivity, specificity and accuracy of combined diagnosis group were 92.4%, 94.5% and 97.6%, existing statistically significant differences with those of color Doppler ultrasound imaging group （P〈0.05）. No significant differences were found in the sensitivity, specificity and accuracy between ultrasound elasticity imaging group and combined diagnosis group （P〉0.05）. Conclusion The ultrasound elasticity imaging can diagnosis the breast solid tumor more accurately. And the diagnostic results have no significant differences with the results of the diagnosis combining ultrasound elasticity imaging and color Doppler ultrasound imaging. So the ultrasound elasticity imaging have important clinical significance.
