高粘度骨水泥椎体成形术治疗骨质疏松性椎体压缩性骨折30例

摘　　要：目的：探析高粘度骨水泥椎体成形术治疗骨质疏松性椎体压缩性骨折的有效性。方法：回顾性分析本院2011年12月—2013年12月收治的30例骨质疏松性椎体压缩性骨折患者的临床资料。患者均采用高粘度骨水泥椎体成形术治疗,比较患者术前、术后1天、术后1月、术后3月及术后1年的椎体前缘高度、椎体中线高度、椎体后缘高度、后凸Cobb角、VAS疼痛评分、ODI脊柱评分,以及骨水泥灌注量和渗漏情况。结果：患者术前的椎体前缘高度、椎体中线高度、椎体后缘高度、后凸Cobb角、VAS疼痛评分、ODI脊柱评分与术后1天、术后1月、术后3月及术后1年的差异显著（P〈0.05）;术后1天、术后1月、术后3月及术后1年的椎体前缘高度、椎体中线高度、椎体后缘高度、后凸Cobb角、VAS疼痛评分、ODI脊柱评分差异不显著（P〉0.05）。此外,30例患者骨水泥灌注量为2.5~6.5m L,平均（3.1±0.4）m L,3例患者出现骨水泥渗漏情况,渗漏率为10%,其中椎旁静脉渗漏、椎间隙渗漏、椎前软组织渗漏各1例。结论：高粘度骨水泥注入椎体成形术治疗骨质疏松性椎体压缩性骨折能快速缓解患者的疼痛程度,且骨水泥渗漏率较低,疗效良好。Objective： To analysis of the effectiveness of high viscosity bone cement in vertebroplasty in treatment of osteoporotic vertebral compression fractures. Methods： A retrospective analysis of our hospital from 2011 December to 2013 December were 30 cases of osteoporotic vertebral compression fractures in patients with clinical data,patients were treated with high viscosity bone cement vertebroplasty treatment,comparison of patients before surgery,1 days after operation,after operation,in January March and after 1 years of anterior vertebral body the height of vertebral body height,midline,vertebral height,kyphosis Cobb angle,VAS pain score,ODI score and the spine,bone cement perfusion and leakage. Results： The preoperative anterior height of vertebral body,vertebral midline height,vertebral height,kyphosis Cobb angle,VAS pain score,ODI spine score and 1 days after operation,postoperative patients after March January,and 1 years after operation the difference was significant,P 0. 05,with statistically significant difference; after 1days,after the operation,after operation in January March and after 1 years of anterior vertebral height,the height of the vertebral midline,vertebral height,kyphosis Cobb angle,VAS pain score,ODI spine score did not show significant difference,P 0. 05,no statistical significance. In addition,30 patients with bone cement perfusion of 2. 5m L ~ 6. 5m L,the average（ 3. 1 ＋ 0. 4） m L,3 patients with bone cement leakage,leakage rate of 10%,of which the paravertebral venous leakage,intervertebral leakage,leakage of the prevertebral soft tissue in 1 cases. Conclusion： High viscosity bone cement vertebroplasty in treatment of osteoporotic vertebral compression fractures can quickly relieve the pain of patients,and bone cement leakage rate is low,good curative effect.
