MRI表观弥散系数用于腰椎间盘退变诊断的价值

摘　　要：目的探讨MRI表观弥散系数值（ADC）用于腰椎间盘退变诊断的价值。方法对50例腰痛患者进行腰椎MRI常规序列和弥散加权成像（DWI）扫描。测量每个腰椎间盘平均ADC值，分析腰椎间盘平均ADC值与退变分级、解剖部位的相关性。结果不同退变分级腰椎间盘平均ADC值差异有统计学意义（P〈0．01），且退变分级越高，其平均ADC值越低；相同退变分级不同解剖部位腰椎间盘平均ADC值均有统计学差异（H=36．71、11．51、21．32，P〈0．05或001），解剖部位与Ⅱ～Ⅳ级ADC值呈正相关（r=0．41、0．40、0．55。均P〈001），越接近足侧腰椎间盘其平均ADC值则越高。结论MRIADC值能够定量评估椎间盘退变，间接反映退变椎间盘微观结构的改变，为临床诊断和治疗提供更多有效信息。Objective To investigate the application of MRI apparent diffusion coefficient （ADC） in evaluation of lum- bar intervertebral disc degeneration. Methods Fifty patients with back pain underwent MRI scan of conventional sequence and diffusion weighted imaging（DWl） on lumbar spine. The mean ADC value was measured for each disc. Relative ADC values of dif- ferent degenerative grades and different anatomic levels of discs were compared, Results Mean ADC values in different grades of all intervertebral discs were of significantly differences （P〈0.001）, with degenerative grade increased the mean ADC values decreased. Statistical differences of mean ADC values were found among all discs of different anatomic levels in the same de- generative grade （P〈0.05）, with higher mean ADC values seen in lower discs. Conclusion The lumbar intervertebral disc de- generation can be evaluated with MRI ADC value quantitatively, which reflects the microstructural changes of discs and provides useful information for clinical diagnosis and treatment.
