高血压前期并糖耐量异常患者颈动脉内中膜厚度变化

摘　　要：目的观察高血压前期合并糖耐量异常患者的颈动脉内中膜厚度变化,并分析独立危险因素。方法将331例35-75岁符合纳入标准的患者进行问卷调查、体格检查、生化检查及双侧颈动脉超声检查,根据JNC-7的血压分类标准,比较对照组、高血压前期组（Pre-HT）、糖耐量异常组（IGT）和高血压前期合并糖耐量异常组（Pre-HT＋IGT）的生化指标变化、平均IMT、颈动脉内径等。结果与对照组相比血压、血脂、尿酸等因素在高血压前期、糖耐量异常组均有一定程度的相关;Pre-HT＋IGT组的颈动脉内中膜厚度明显增加,颈动脉内径缩小（P〈0.05）;Pre-HT组与IGT组之间的颈动脉内中膜厚度差异无统计学意义（P〉0.05）。结论高血压前期合并糖耐量异常的患者出现颈动脉内中膜增厚、颈动脉狭窄的风险明显增加。【Objective】Observe the change of the carotid intima-media thickness（IMT） in patients with pre-hypertension complicated with glucose tolerance, and analyze the independent risk factors.【Method】According to the criterion of JNC-7,331 patients at the age of 35-75 met the inclusion criteria were divided into control group, the pre-hypertension（PreHT） group, impaired glucose tolerance（IGT） group and pre-hypertension complicated with abnormal glucose tolerance（PreHT＋IGT） group,each group conducted a questionnaire survey, physical examinations, biochemical tests and bilateral carotid ultrasound examination.The biochemical indexes of average IMT, carotid diameter and biochemical reslut in different groups were statistical analyzed.【Results】Compared with the control group,the blood pressure, blood lipid, uric acid and some other factors in the pre-HT group, IGT group and Pre-HT＋IGT group showed correlation in certain degree. Intima-media thickness of carotid artery in Pre-HT＋IGT group increased significantly, the diameter of the carotid artery was narrowing（P〈0.05）; no significant differences were observed in carotid artery intima-media thickness between the Pre-HT group and IGT group（P〉0.05）.【Conclusion】The risk of carotid intima-media thickness, carotid artery stenosis were increased significantly in patients with pre-hypertension complicated with abnormal glucose tolerance.
