三种巨乳缩小术的术后并发症及远期疗效比较

摘　　要：目的比较三种巨乳缩小术的术后并发症及远期疗效。方法选取我院行巨乳缩小术的患者46例作为研究,根据手术方式分为3组,A组12例行垂直双蒂瓣乳房缩小成形术,B组15例行乳房无垂直切口的横双蒂瓣缩小成形术,C组19例行横双蒂真皮帽乳房缩小成形术,观察3组患者的远期疗效,记录术后并发症发生率。结果 3组患者术后远期疗效无明显差异,不具统计学意义(P>0.05);3组患者伤口裂开、脂肪液化的发生率无明显差异(P>0.05);A组患者术后乳晕感觉缺失、瘢痕增生概率均为25.0%略高于B组、C组,但差异无统计学意义(P>0.05)。结论三种巨乳缩小术远期疗效相同,但无垂直切口的横双蒂瓣乳房缩小成形术术后无明显瘢痕组织,易被重度乳房肥大患者接受。【Objective】To compare the postoperative complications and prostecdtive efficacy of three kinds of reduction mammaplasty. 【Method】Chose 46 cases of patients treated with reduction mammaplasty in our hospital, divided into three groups according to the operation method, 12 cases in group A took vertical double pedicle flap breast reduction surgery, 15 cases in group B took no vertical incision of transverse double pedicle flap breast reduction surgery, 19 cases in group C took transverse double pedicle leather cap breast reduction surgery, observed the prostecdtive efficacy of patients in three groups and recorded the incidence of postoperative complications. 【Results】The postoperative prostecdtive efficacy of three groups had no significant difference, with no statistical significance（P〉0.05）; The incidence of wound dehiscence and fat liquefaction of the three groups had no significant difference（P〉0.05）; The rate of mammary areola anaesthesia and hypertrophic scar were 25.0, higher than group B and C, but the difference was no statistical significance（P〉0.05）. 【Conclusion】The prostecdtive efficacy of three kinds of reduction mammaplasty were the same, but no vertical incision of transverse double pedicle flap breast reduction surgery had no obvious scar tissue after the operation, which can easily accepted by the patients with severe breast hypertrophy.
