摘 要:目的探讨血清降钙素原(PCT)定量测定在新生儿血流感染诊断中的临床价值。方法选取2014年1月-2014年12月本院新生儿病房收治的血培养阳性患儿82例,作为血流感染组;选取同一时段新生儿血培养阴性患儿53例,作为非血流感染组。用SPSS 19.0软件比较血流感染组和非血流感染组血清PCT水平差异,同时绘制ROC曲线,评估血清PCT水平在新生儿血流感染诊断中的应用价值。结果新生儿血流感染组和非血流感染组之间血清PCT水平差异具有统计学意义(P<0.01),ROC曲线面积为0.826,以0.445 ng/ml作为诊断的cut-off值时,血清PCT水平诊断新生儿血流感染的灵敏度为87.8%,特异度为71.7%。结论血清PCT定量测定可作为新生儿血流感染较有价值的诊断指标。Objective To investigate the clinical value of serum level of the quantitative determination of serum procalcitonin( PCT) in the diagnosis of neonatal bloodstream infection. Methods 82 neonatal patients suffered from bloodstream infection,confirmed with blood culture,were selected as a group of bloodstream infection,compared with 53 cases with negative blood culture as non bloodstream infection group,from January 2014 to December 2014 in neonatal ward in our hospital. SPSS 19. 0 software was applied to compare the difference of serum PCT level in patients with bloodstream infection and non blood flow infection,and to evaluate the value of ROC in the diagnosis of neonatal bloodstream infection. Results It had statistical significance on the neonatal serum PCT level difference between the bloodstream infection group and non bloodstream infection group( P〈0. 01),when the area under ROC curve was 0. 826,0. 445 ng / ml as a cut- off value,the sensitivity of serum levels of PCT in the diagnosis of neonatal blood infection was 87. 8% and the specificity was 71. 7%. Conclusion Serum PCT can be used as a diagnostic index of neonatal blood flow infection.
