摘  要:目的分析HIV抗体不确定样本的血清学特征,预测抗体进展情况,探讨不确定样本的应对方案。方法分析104例HIV抗体不确定者的WB条带特点及16例随访病例的初筛结果、病毒载量结果、受检者随访情况、抗体转归情况。结果 WB有12种带型,p24和gp160p24带型最常见。随访成功率为15.4%。13例转阳,首次S/CO>6,病毒载量可检出。3例为阴性,首次S/CO<6。结论 ELISA的S/CO高、硒标阳性且带型含gp160,条带深的样本阳转率高,gp160p24带型者可缩短随访时间,以增加随访率。ELISA的S/CO值、硒标结果、WB带型有助于分析患者的感染状态,预判断抗体转归趋势,对可疑病例重点关注,减少随访期间的二次传播,必要时可增加病毒载量和PCR检测,及早明确诊断。Objective To analyze the serological characteristics of HIV antibody indeterminate results,and predict the progress of the antibody,to explore the solutions of indeterminate samples. Methods WB band patterns of 104 antibody indeterminate cases were studied. 16 cases were followed up,the results of the initial screening,the viral load,the follow- up of the patients,and the prognosis of the antibody were also studied. Results WB had 12 kinds of band patterns,p24 and gp160p24 were most common. The success rate of follow- up was 15. 4%. 13 cases finally converted to be HIV- positive,the first time S / CO 6,and viral load can be detected. 3 cases were confirmed to be negative,the first time S / CO 6. Conclusion Samples with high ELISA S / CO,selenium labeled positive,contained gp160 band had higher converted rate to be positive. The follow- up time of cases with gp160p24 band pattern can be shorten appropriately to increase the rate of follow- up. ELISA S / CO,selenium test result,WB patterns can help to analyze the infection state of patients and judge the trend of antibody outcome,thus suspected cases can be focused on to reduce the secondary transmission,when necessary,viral load assay and PCR can be used in the early diagnosis.
