摘  要:目的探讨儿童急性白血病(AL)骨髓细胞中Notchl蛋白表达及其与免疫分型的关系。方法收集2009年10月至2011年11月初诊的42例AL患儿和22例正常或非恶性血液病儿童(对照组)骨髓涂片,采用免疫组织化学方法检测骨髓中Notch1蛋白表达情况。结果 42例AL患儿中,急性淋巴细胞白血病(ALL)32例,其急性B淋巴细胞白血病(B-ALL)25例,急性T淋巴细胞白血病(T-ALL)7例;急性髓细胞白血病(AML)10例。Notch1蛋白在儿童急性白血病中的表达情况,AML组表达率为60.0%(6/10),T-ALL组为71.4%(5/7),B-ALL组为28.0%(7/25),对照组为13.6%(3/22)。与对照组相比,AML组和T-ALL组Notch1蛋白表达率均明显增高,差异均有统计学意义(x^2值分别为7.311、8.878,均P<0.05);B-ALL组Notch1蛋白表达率亦高于对照组,但差异无统计学意义(P>0.05)。T-ALL患儿和B-ALL组比较,差异有统计学意义(x^2=4.400,P<0.05)。Notch1蛋白阳性细胞表达率按照ALL-LR、ALL-MR、ALL-HR依次升高,HR组高于LR组及MR组,但组间比较,差异均无统计学意义(P>0.05)。结论 Notch1蛋白在不同类型的儿童AL中表达有明显差异,T-ALL发病与异常的Notch1信号有关。Notch1信号可能在AL中起着重要作用,但需要进一步收集资料论证。Objective To study the expression of Notch1 protein in children with acute leukemia ( AL) and to explore the relationship of Notch1 expression with immunophenotype.Methods The bone marrow smears of 42 cases of AL and 22 healthy children or cases of nonmalignant hematologic disease were collected during October 2009 to November 2011.Immunocytochemical assay was used to detect the expression of Notch1 protein in the cases.Results Of 42 cases of AL, there were 32 cases of acute lymphocytic leukemia ( ALL) including 25 cases of B-ALL and 7 cases of T-ALL, and 10 cases of acute myelogenous leukemia ( AML) .The expression rate of Notch1 protein was 60.0%(6/10), 71.4%(5/7), 28.0%(7/25)and 13.6%(3/22)in AML group, T-ALL group, B-ALL group and the control group, respectively.Compared with the control group, the expression rate of Notch1 protein in AML group and T-ALL group was remarkably increased, and the differences were significant (χ^2 value was 7.311 and 8.878, respectively, both P〈0.05).The expression rate of Notch1 protein was higher in B-ALL group than in the control group, but the difference was not significant (P〉0.05).The difference in the expression of Notch1 protein between T-ALL group and B-ALL group was not significant (χ^2 =4.400,P〈0.05) .The expression rate of Notch1 protein positive cells increased gradually in ALL-LR group, ALL-MR group and ALL-HR group, and that was higher in HR group than in LR group and MR group.But the comparison between groups showed no significant difference (P〉0.05).Conclusion There are significant differences in the Notch1 protein among children with different types of AL, and abnormal expression of Notch1 is correlated with the occurrence of T-ALL.Notch1 signals might play important role in AL, which needs further researches to confirm.
