摘  要:目的 探讨新型声触诊组织成像和定量(VTIQ)剪切波弹性成像技术诊断BI-RADS 4类乳腺病灶良恶性的价值。方法 回顾性分析95个BI-RADS 4类乳腺结节的常规超声及VTIQ图像资料,获得病灶内部剪切波速度(SWV)的平均值。分析良恶性病灶剪切波速度平均值之间的差异。以病理结果为金标准,利用受试者操作特征(ROC)曲线评价VTIQ鉴别乳腺可疑恶性病灶的价值。结果 病理结果证实良性病灶59个,恶性病灶36个。良恶性病灶VTIQ上SWV平均值分别为为(3.03±0.86)m/s(1.35-5.75 m/s)和(5.09±1.52)m/s(2.56-8.59 m/s),差异有显著性意义(P〈0.001)。绘制受试者操作特征(ROC)曲线得到SWV值3.49 m/s为良恶性诊断最佳界点,诊断BI-RADS 4类乳腺结节良恶性的敏感性、特异性、准确性、阳性预测值、阴性预测值分别为94.4%(34/36)、81.4%(48/59)、86.3%(82/95)、75.6%(34/45)、96.0%(48/50)。结论 VTIQ技术对常规超声发现的乳腺BI-RADS 4类实性病灶的鉴别诊断有较好的应用价值。Objective To assess the diagnostic value of the novel virtual touch tissue imaging quantification (VTIQ) technique of acoustic radiation force impulse (ARFI) elastography in differentiating benign and malignant breast lesions with Breast Imaging Reporting and Data System (BI-RADS) 4 classification on conventional ultrasound (US). Methods The US and VTIQ of 95 pathologically proven breast lesions were retrospectively analyzed. The average shear wave velocity (SWV) was obtained from multiple SWV measurements under the VTIQ speed mode. Receiver operating characteristic (ROC) curves were plotted to determine the cut-off value of VTIQ for different diagnosis of BI-RADS lesions. Results The mean values of SWV on VTIQ were 3.03 ±0.86 m/s (range: 1.35-5.75 m/s) for benign lesions and were significantly lower than the 5.09±1.52 m/s (range: 2.56-8.59 m/ s) for malignant lesions (P〈0.001). At the cut-off SWV of 3.49 m/s, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of BI-RADS 4 lesions were 94.4% (34/36), 81.4% (48/59), 86.3% (82/95), 75.6% (34/ 45), and 96.0% (48/50), respectively. Conclusion VTIQ measuring the stiffness is a reliable method for differentiating benign and malignant BI-RADS 4.
