摘　　要：目的利用收益一成本比（benefit-cost ratio，BCR）评估甘肃省畸变产物耳声发射（DPOAE）联合自动听性脑干反应（AABR）进行新生儿听力筛查以及听力医疗救助的经济效益，为听力学家、管理人员以及参与新生儿普遍听力筛查的其他人员提供参考依据。方法出生后48～72小时应用DPOAE及AABR进行初次听力筛查，任意一项未通过者干出生后42天进行DPOAE联合AABR复筛，未通过者于3月龄时进行听力学诊断性检查，从而获得新生儿先天性听力损失的发病率，结合甘肃省卫生部门财政投入估算费用及货币化收益来计算BCR。结果①每例新生儿筛查成本为49元；②每例听力障碍患儿筛查成本为25466元；③每例中度以上听力障碍患儿筛查成本为48063元；④通过成本和效益估算表明BCR=8．11。结论甘肃省新生儿听力筛查与救助项目获得的收益超过成本，该项目符合收益一成本的原则。Objective To evaluate the economic efficiency of newborn hearing screening with DPOAE and AABR and medical assistance in Gansu province by benefit-cost ratio（BCR） and to provide reference for audiologists, administrative staff and other staff in the universal newborn hearing screening（UNHS） program. Methods In the initial screening,the distortion product otoacoustic emission （DPOAE） and automated auditory brainstem response （AABR） were performed to the newborns 48-72 hours after birth. The newborns who failed either of the tests received the DPOAE and AABR test 42 days after birth in the second screening. The children who failed the second screening received the audiological tests 3 months after birth. The prevalence of congenital hearing loss in newborns was obtained and the BCR was calculated by the cost and monetary benefits assessed from the financial investment of the health department in Gansu province. Results （1）The cost of hearing screening for each newborn was 49 RMB.（2） The cost of hearing screening for each hearing-impaired newborn was 25466 RMB. （3）The cost of hearing screening for each newborn with moderate or more severe hearing loss was 48063 RMB. （4）The benefit-cost estimation showed BCR was 8.11. Conclusion The benefits of newborn hearing screening and medical assistance program exceed the cost, so this program is in accordance with the benefit-cost principle.
