摘 要:目的:探讨UF-100尿沉渣分析、干化学分析和尿液培养在尿路感染诊断中的意义。方法:对某市某医院658份中段尿标本做细菌培养和沉渣分析、干化学分析,并比较结果。结果:尿细菌培养阳性235例,阳性率为35.7%;与尿细菌培养比较,UF-100及US-2100R不同组合对尿路感染的诊断价值如下:假阳性率为4.49%~13.10%;假阴性率为20.41%~24.01%;敏感性32.77%~44.00%;特异性79.63%~93.23%;阳性预测值为49.68%~71.21%;阴性预测值为68.59%~73.39%;准确率64.13%~73.05%。结论:在尿路感染的实验室检查中,UF-100和US-2100R结果阳性时,对尿路感染有一定的预见性,但不能肯定尿路感染,仍须进行细菌培养以进一步确认;结果阴性时,也无法完全排除尿路感染,也就是干化学及尿沉渣结果无法明确预见尿细菌培养结果,诊断尿路感染仍需尿细菌培养。Objective:To explore the clinic value about combined flow cytometry (Sysmex UF-100) and dipsticks and urine cultures in diagnosis of urinary tract infection. Methods:658 urine samples were collect from the patients who suspected urinary tract infection, samples were tested with dipsticks, the flow cytometry and cultures, then compare all results.Results:Urinary cultures identified 235 (35.7%) samples had a pure growth of more than 10(5)cfu/ml, compared with cultures, the two methods of flow cytometry (Sysmex UF-100) and dipsticks showed a result as follows:the false positive rate(FPR) was 4.49%~13.10%;the false negative rate(FNR) was 20.41%~24.01%; the sensitivity was 32.77%~44.00%; the specificity was 79.63%~93.23%; the positive predictive value was 49.68%~71.21%;the negative predictive value was 68.59%~73.39%;the accurate rate was 64.13%~73.05%. Conclusions:Flow cytometry (Sysmex UF-100) and dipsticks (Sysmex US-2100R) ’s positive rusults performed a certain predictability in urinary tract infection, but negative rusults detected by flow cytometry (Sysmex UF-100) and dipsticks (Sysmex US-2100R) also can ’t exclude urinary tract infection, whether urinary tract infections requiring the detection of urinary culture.
