摘　　要：目的研究鲍氏不动杆菌在医院分布及耐药性变迁,为临床治疗提供试验依据。方法回顾性分析2006年1月-2012年12月医院3 253株鲍氏不动杆菌分布及其耐药性,采用全自动微生物分析仪VITEK-32、2Compact进行细菌鉴定,药敏试验采用K-B法;使用WHONET5.6软件对数据进行统计分析。结果 2006-2012年鲍氏不动杆菌分离菌株逐年上升,从2006年195株升至2012年的806株,主要分离自下呼吸道占77.6%;以ICU、外科ICU、呼吸科ICU、肾病科检出率最高,分别占24.3%、20.0%、10.1%、7.3%;2006-2012年鲍氏不动杆菌对19种抗菌药物的耐药率呈逐年上升趋势,2006年亚胺培南、美罗培南耐药率分别为8.7%和7.7%,急速升至2012年的64.6%和68.4%。结论连续7年鲍氏不动杆菌对19种抗菌药物的耐药率逐年上升,给临床抗感染治疗带来很大困难,应重视耐药菌株监测。OBJECTIVE To investigate the clinical distribution and change of drug resistance of Acinetobacter baumannii strains so as to provide experimental basis for clinical treatment.METHODS From Jan 2006 to Dec 2012,the distribution and drug resistance of 3 253 strains of A.baumannii were retrospectively analyzed,then the bacterial identification was carried out by using VITEK-32 compact automatic microorganism analyzer,the drug susceptibility testing was conducted by means of K-B method,and the statistical analysis of data was performed with the use of WHONET5.6software.RESULTS The A.baumannii isolates were increased year by year from 2006to2012,increasing from 195 strains in 2006 to 806strains in 2012.77.6% of the A.baumannii strains were isolated from the lower respiratory tract.The isolation rate of the A.baumannii strains was 24.3%in the ICU,20.0%in the ICU of surgery department,10.1% in the ICU of respiratory department,7.3% in the nephrology department.The drug resistance rates of the A.baumannii strains to 19 antibiotics showed upward trends from 2006to2012,the drug resistance rates to imipenem and meropenem were respectively 8.7% and 7.7%in 2006,increasingly rising to 64.6% and 68.4%in 2012.CONCLUSIONThe drug resistance rates of the A.baumannii strains to the 19 antibiotics showed upward trends during the 7consecutive years,which brought great difficulties to the clinical anti-infection treatment.It is necessary to pay attention to the monitoring of the drug-resistant strains.
