摘 要:目的探讨白内障患者围手术期抗菌药物使用不同时间和剂量对术后医院感染率的影响,为临床用药提供参考。方法选取术后给药方法相同的白内障手术患者2 521例,其中A组847例,术前1d给药8次;B组855例,手术当天术前给药8次;C组819例,术前不给药;选取术前给药方法相同的白内障手术患者1 059例,术后每天用药均为4次,其中a组352例,术后用药3d;b组359例,术后用药7d;c组348例,术后用药14d;选取术前用药1d,术后用药3d的患者1 121例,小剂量组372例,术前、术后用药均每天4次;中剂量组381例,术前每天8次,术后每天4次;大剂量组,手术前后均每天8次。结果术后A、B、C组患者医院感染率分别为0.50%、2.22%和9.91%,A、B两组差异无统计学意义,A、B两组均明显低于C组,且差异有统计学意义(P<0.05);术后a、b、c组患者医院感染率分别为1.42%、1.11%和4.89%,a、b两组差异无统计学意义,a、b两组均明显低于c组,差异有统计学意义(P<0.05);术后小量组、中量组、大量组患者医院感染率分别为4.03%、0.79%和0.82%,中、大量两组差异无统计学意义,中、大量两组均明显低于小量组,差异有统计学意义(P<0.05)。结论术前1d8次给药,术后3d4次给药,可以有效预防白内障术后感染的发生,且不增加患者负担,是一种较为理想的给药方案。OBJECTIVE To investigate effects of the perioperative use of antimicrobial agents in different time and dosages on the incidence of nosocomial infection after cataract surgery,so as to provide reference for clinical use.METHODS Totally 2521 cataract cases who received the same means of administration after surgery were selected,including 847cases(group A)administered 8times one day before surgery,855patients(group B)administered 8times preoperatively at the same day of surgery,819cases(group C)not administered preoperatively.The 1059 cataract cases who received the same means of administration before surgery were selected.They were all administrated 4times a day after surgery,including 352cases(group a)administrated at postoperative 3d,359cases(group b)administrated at postoperative 7d,348cases(group c)administrated at postoperative 14 d.The 1121 cases administrated at preoperative 1dand postoperative 3dand the low-dose group of 372 cases were selected and they were administrated 4times a day both preoperatively and postoperatively.The medium-dose group of 381 cases was administrated 8times a day preoperatively and 4times a day postoperatively.The high-dose group was administrated 8times day both preoperatively and postoperatively.RESULTS The postoperative nosocomial infection rates were 0.50%,2.22% and 9.91%in group A,group B and group C.There was no significant difference between group A and group B,while the difference was significant between group A,group B and group C as the nosocomial infection rate was lower in groups A and B than in group C(P〈0.05).The postoperative nosocomial infection rates were 1.42%,1.11% and 4.89%in group a,group b and group c.There was no significant difference between group a and group b,while the difference was significant between group a,group b and group c as the nosocomial infection rate was lower in groups a and b than in group c(P〈0.05).The postoperative nosocomial infection rates were 4.03%,0.79% and 0.82%in low-dose,medium-dose and high-d
