摘 要:目的了解医院经外周置入中心静脉导管(PICC)相关性血流感染状况,为有效预防PICC导管相关性血流感染提供参考。方法选取医院2014年1-12月收治的50例PICC导管相关性血流感染患者为研究对象,分析感染患者发生PICC导管相关性血流感染的感染部位分布、病原菌分布及相关危险因素。结果患者年龄与PICC导管相关性血流感染关系密切,46-55岁年龄段患者感染例数最多,占52.0%;感染类型以导管相关性血流感染所占比率最大,占56.00%;共检出病原菌50株,其中金黄色葡萄球菌20株占40.0%,肺炎克雷伯菌15株占30.0%,其他菌类仅占10.0%;PICC导管相关性血流感染与操作人员经验、穿刺部位、导管滞留时间、穿刺次数以及是否正规换药具有相关性(P〈0.05),其中,操作人员经验≤50次的发生率高达80.0%,穿刺部位在左贵要静脉的发生率最大,为40.0%,导管滞留时间〉30d发生率最高,达94.0%。结论 PICC导管相关性血流感染因素多,规范PICC的管理是预防PICC导管相关性血流感染的有效措施。OBJECTIVE To discuss the status of PICC related bloodstream infections in our hospital so as to provide reference for effective prevention of PICC related bloodstream infections.METHODS Totally 50 cases of patients with PICC catheter related bloodstream infections from Jan.to Dec.2014 were selected as research objects.The infection sites,pathogenic distribution and related risk factors were analyzed.RESULTS The study showed that age was the most relevant to PICC related bloodstream infections.We also found that patients aged 46 to 55were the most susceptible to infections,accounting for 52.0%.PICC related bloodstream infections accounted for the majority of infections,holding 56.00%.Totally 50 strains of pathogens were detected,with 20 strains of Staphylococcus aureus,taking up 40.0%,followed by Klebsiella pneumonia with 15 strains accounted for 30%.Other pathogens only held 10.0%.PICC related bloodstream infection is relevant to experience of the operating staff,puncture sites,catheter dwelling time,frequency of puncture and standard dressing change(P〈0.05).Among them,the infection rate in patients received treatments operated by staff with practice under 50 times reached up to80.0%.And patients with left basilica vein puncture were easy to get infected and the infection rate was 40.0%.And those received more than 30 days of catheter dwelling had the highest infection rate,which reached up to94.0%.CONCLUSION Various factors are relevant to PICC catheter related infection.Standard management of PICC is an effective measure to prevent PICC related bloodstream infections.
