改良主干剥脱术降低下肢静脉曲张患者医院感染的临床研究

摘　　要：目的探讨改良主干剥脱术降低下肢静脉曲张患者医院感染的效果,为临床治疗提供参考。方法选取2010年2月-2013年12月于医院治疗下肢静脉曲张的120例患者为观察对象,随机分为观察组和对照组,各60例;观察组采用改良主干剥脱术,即高位结扎加点式剥脱术,结合驱血带,先小腿后大腿剥脱;对照组则用传统的根治手术方法,即静脉结扎加剥脱,比较观察两种治疗方法的疗效。结果观察组与对照组疗效比较,观察组的60例患者中,33条患肢治愈、37条患肢好转、1条患肢无效;对照组60例患者中,28条患肢治愈、33条患肢好转、4条无效无效,差异无统计学意义;两组术后相关指数比较,观察组患者患肢的手术时间(41.2±6.3)min,切口数(3.1±0.2)个,1条患肢发生感染;对照组患者患肢手术时间(55.3±11.2)min,切口数(6.4±1.4)个,4条患肢发生术后感染;观察组患者患肢手术时间短、切口个数少、感染率低,效果均优于对照组,两组差异有统计学意义(P<0.05)。结论大隐静脉高位结扎加点式剥脱术,相对于传统方法,具有操作简单、切口小、恢复快、术后患者生活质量较优等优点。OBJECTIVE To investigate the improvement of varicose veins of lower limb by modified trunk exfoliation so as to provide reference for clinical treatment.METHODS Totally 120 patients with varicose veins of lower limb in our hospital during Feb.2010 to Dec.2013 were selected and randomly divided into the observation group and the control group of 60 cases in each group.The observation group used modified trunk exfoliation,namely,high ligation point endarterectomy combined with tourniquet for shank followed by thigh.The control group used traditional methods of radical surgery,namely,vein ligation and stripping.The efficacy of two treatments was compared.RESULTS Totally 33 cases were cured,37 improved and 1invalid in the 60 patients in the observation group;28cured,33 improved and 4invalid in the control group.The difference was not significant in efficacy.The limb surgery time was（41.2±6.3）min,the number of incisions was（3.1±0.2）,and 1limb was infected in the observation group.The limb surgery time was（55.3±11.2）min,the number of incisions was（6.4±1.4）,and four limbs were infected in the control group.The patients experienced shorter operative time,less number of cuts and low infection rate in the observation group,superior than the control group,the difference was significant（P〈0.05）.CONCLUSIONCompared with the traditional method,the large saphenous vein high ligation point endarterectomy has advantages of simple operation,small incisions,quicker recovery,and the postoperative quality of life of patients is better.
