子宫内膜癌患者与人乳头状瘤病毒感染的相关性研究

摘　　要：目的了解人乳头状瘤病毒（HPV）,并探讨HPV52与HPV58感染与子宫内膜癌发生的相关性,了解其感染途径,预防其感染。方法选择2012年1月-2015年2月在医院进行子宫病变筛查的2 830名女性为研究对象,统计2 830名受检女性HPV的感染率,分析受检女性年龄分布与HPV感染的关系;比较几种高危险型HPV的阳性率;观察几种HPV与宫内病变的关系,最后观察HPV52与HPV58与子宫内膜癌发生的关系。结果年龄30-39岁的女性HPV的感染率最高,为21.66%,年龄20-29岁的女性HPV感染率最低,为7.40%,各年龄段感染率差异有统计学意义（P〈0.05）;感染HPV的人群中,亚型为HPV16的感染率最高,为37.7%,其次HPV52和HPV58,感染率分别为18.4%和16.7%,差异有统计学意义（P〈0.05）;经液基薄层细胞检测（TCT）结果显示,HPV16更容易导致子宫肌瘤、子宫内膜癌等子宫病变,其次为HPV52和HPV58,且不同类型HPV所导致的子宫病变差异有统计学意义（χ2=60.4304,P〈0.01）;HPV病毒导致的子宫内膜癌中,HPV52占26.67%,HPV58占22.67%,差异有统计学意义（P〈0.05）。结论 HPV52和HPV58感染与子宫内膜癌的发生具有相关性,预防HPV病毒感染对于预防子宫内膜癌的发生具有重要意义。OBJECTIVE To understand the human papillomavirus（HPV）,explore the correlation between the HPV52,HPV58 infection and the endometrial cancer,and observe the routes of infections so as to prevent the infections.METHODS A total of 2 830 women who underwent the screening of uterine diseases from Jan 2012 to Feb2015were recruited as the study objects.The incidence rates of HPV infections in the 2 830 women were statistically analyzed,the relationship between the age of the subjects and the HPV infection was observed.The positive rates of high-risk HPV were compared;the correlation between the HPV infection and the uterine diseases was analyzed,and the relationship between the HPV52,HPV58 and the endometrial cancer was finally observed.RESULTS The incidence rate of HPV infection was the highest（21.66%）in the women aged between 30 and 39years old,while the incidence rate of HPV infection was the lowest（7.40%）in the women aged between 20and29 years old,and there was significant difference in the infection rate between the two age groups（P〈0.05）.Among the population with HPV infection,the incidence rate of HPV infection was the highest（37.7%）,followed by the incidence rate of HPV52infection（18.4%）and the incidence rate of HPV58infection（16.7%）,with significant difference（P〈0.05）.The results of the liquid-based cytology test showed that the HPV16 was more likely to result in the uterine diseases such as myoma of uterus and endometrial cancer than the HPV52 and HPV58,and there was significant difference in the uterine diseases induced by various subtypes of HPV（χ2=60.4304,P〈0.01）.Of the women with endometrial cancer included by the HPV,26.67% had the cancer induced by the HPV52,and 22.67% had the cancer induced by the HPV58, with significant difference（P 0.05）.CONCLUSIONThe incidence of the endometrial cancer is associated with the HPV52 and HPV58infections.It is of great significance to prevent the HPV infection so as to prevent the endometrial cancer.
