摘 要:目的分析胎心监护分级中Ⅱ类图形联合脐动脉血流异常孕妇的妊娠结局。方法选取2011年1月~2012年12月在该院分娩的孕妇800例,按胎心监护图形分为Ⅰ类图形组和Ⅱ类图形组,Ⅱ类图形组患者按脐动脉血流情况分为正常组和异常组,比较分析各组妊娠结局。结果Ⅱ类图形组新生儿死亡率、羊水污染率、羊水少率、脐带异常率、剖宫产率和胎盘老化率显著高于Ⅰ类图形组(χ^2=4.19,P〈0.05;χ^2=8.37,P〈0.01;χ^2=14.59,P〈0.01;χ^2=22.98,P〈0.01;χ^2=67.71,P〈0.01;χ^2=13.62,P〈0.01);Ⅱ类图形组各种胎心监护异常妊娠结局特征也不同;Ⅱ类图形脐动脉血流异常组羊水少率、新生儿窒息率、脐带异常率和剖宫产率均显著高于正常组(χ^2=9.20,P〈0.01;χ^2=7.941,P〈0.01;χ^2=20.53,P〈0.01;χ^2=15.74,P〈0.01)。结论Ⅱ类图形联合脐动脉血流异常可导致更多的不良妊娠结局,应及早做好应对措施。Objective To analyze the pregnancy outcome of pregnant women with type II graphics during fetal monitoring classification combined with abnormal umbilical artery blood flow. Methods Eight hundred pregnant women giving birth to their babies in the hospital from January 2011 to December 2012 were selected and divided into type I graphics group and type II graphics group according to fetal heart rate monitoring; the pregnant women in type II graphics group were divided into normal group and abnormal group according to umbilical artery blood flow, the pregnancy outcomes in different groups were compared and analyzed. Results Neonatal mortality, the incidence rates of amniotie fluid contamination, oligohydramnios, abnormality of umbilical cord, cesarean section, and placental aging in type II graphics group were statistically significantly higher than those in type I graphics group (χ^2 = 4. 19, P〈0.05 ; χ^2 = 8.37, P〈0. 01 ; χ^2 = 14. 59, P〈0. 01 ; χ^2 = 22. 98, P〈0. 01 ; χ^2 = 67.71, P〈0. 01 ; χ^2 = 13.62, P〈0. 01 ) ; in type II graphics group, the characteristics of pregnancy outcome varied in pregnant women with different abnormalities of fetal heart rate monitoring; the incidence rates of oligohydramnios, neonatal asphyxia, abnormality of umbilical cord, and cesarean section in type II graphics group were statistically significantly higher than those in normal group (χ^2=9.20, P〈0.01;x2=7.941, P〈0. 01; χ^2 =20.53, P〈0.01; χ^2 =15.74, P〈0.01) . Conclusion Type II graphics during fetal monitoring classification combined with abnormal umbilical artery blood flow can induce more adverse pregnancy out-come, corresponding measures should be taken as soon as possible.
