复律宁颗粒辅助治疗气阴两亏型慢性心力衰竭合并心律失常30例临床观察

摘　　要：目的评价复律宁颗粒辅助治疗气阴两亏型慢性心力衰竭合并心律失常的临床疗效和安全性。方法60例中医辨证为气阴两亏型的慢性心力衰竭合并心律失常患者随机分为治疗组和对照组各30例。对照组给予西医规范化治疗,治疗组在对照组治疗基础上加用复律宁颗粒,每次15 g,每日2次。两组疗程均为4周。观察两组患者NYHA心功能分级、六分钟步行距离、心脏彩色超声、动态心电图、明尼苏达心衰生活质量(LHFQ)评分(包括体力、情绪、社会、经济)、中医证候评分及安全性,治疗后评价心功能疗效和心律失常疗效。结果治疗后两组心功能疗效(治疗组总有效率76.67%,对照组为50.00%)及心律失常疗效(治疗组总有效率66.67%,对照组为30.00%)比较差异均有统计学意义(P<0.05或P<0.01),治疗组优于对照组。治疗后两组中医证候积分,LHFQ体力、经济、社会评分及总积分均较治疗前降低,而六分钟步行距离提高(P<0.01);并且治疗后治疗组中医证候积分,LHFQ体力、经济、社会评分及总积分低于对照组,六分钟步行距离高于对照组(P<0.05)。两组治疗前后心脏彩超各指标比较差异均无统计学意义(P>0.05)。结论复律宁颗粒辅助治疗气阴两亏型慢性心力衰竭患者可改善心功能和生活质量,安全有效。Objective To evaluate clinical curative effect and safety of Fulvning Granule as an adjuvant therapy in treating 30 cases of chronic heart failure with arrhythmia with Qi and Yin deficiency. Methods Sixty cases of chronic heart failure with arrhythmia with qi and yin deficiency according to Chinese Medicine syndrome differentiation were randomly divided into a treatment group and a control group, with 30 cases in each. Control group was given Western medicine standardized treatment, while treatment group used Fulvning Granule additionally on the base of the treatment of control group, 15 g each time, twice a day. Courses of the treatment of two groups were both 4 weeks. The New York Heart Association （NYHA） cardiac functional grading, Six-Minute Walk Distance, cardiac color ultra- sound, dynamic electrocardiogram, Minnesota Living with Heart Failure Questionnaire （LHFQ） scores （including physical, emotional, economic, and social items） , traditional Chinese medicine （TCM） syndrome scores, and treatment safety of patients in two groups were observed. Then, the curative effect on cardiac function and arrhythmia was evaluated. Results Curative effects on cardiac function （the total effective rates was 76. 67% in treatment group, and 50. 00% in control group） and arrhythmia （the total effective rates was 66. 67% in treatment group, and 30. 00% in control group） in two groups after treatment were significantly different （ P 〈 0.05 or P 〈 0. 01 ）. Treatment group was superior to control group. After treatment, TCM syndrome scores, LHFQ physical, economic, and social scores, as well as the total LHFQ scores decreased, while Six-Minute Walk Distance increased in both groups （P 〈0. 01 ）, especially in the treatment group （P 〈 0. 05 ）. There was no statistically significant difference in all indexes of cardiac color ultrasound between two groups before and after treatment （ P 〉 0. 05 ）.
