摘 要:目的探讨甲泼尼龙琥珀酸钠鞘内注射对大鼠急性脊髓损伤后生长相关蛋白-43(GAP-43)表达的影响。方法实验分为治疗组、观察组、对照组,每组30只。治疗组予以脊髓损伤后鞘内注射3 mg·kg-1甲泼尼龙琥珀酸钠+0.9%氯化钠0.1 m L;观察组予以单纯手术暴露脊髓,不损伤脊髓;对照组予以脊髓损伤后鞘内注射0.9%氯化钠0.1 m L。用免疫组织化学方法检测脊髓损伤后各组不同时段脊髓前角细胞中表达GAP-43的细胞个数。各组大鼠在存活期间作后肢Basso Beattie Bresnahan(BBB)运动功能评分。用苏木精-伊红染色法观察各组脊髓组织病理变化。结果免疫组织化学方法结果显示,甲泼尼龙琥珀酸钠鞘内注射增加了脊髓前角细胞GAP-43的表达。BBB评分结果提示,甲泼尼龙琥珀酸钠鞘内注射可改善急性脊髓损伤后肢大鼠运动功能。苏木精-伊红染色结果显示,甲泼尼龙琥珀酸钠鞘内注射可减轻脊髓损伤后的病理变化。结论甲泼尼龙琥珀酸钠鞘内注射可以促进损伤脊髓前角细胞修复,其作用机制可能与甲泼尼龙琥珀酸钠鞘内注射增强损伤脊髓前角细胞GAP-43的表达有关。Objective To research the effect of intrathecal injection of methylprednisolone sodium succinate (MPss) on growth associated protein- 43 (GAP-43 ) expression in rats after acute spinal cord injury (SCI). Methods The rats were divided into the treatment group, the observation group and the control group, with 30 rats in each group. The rats undergone SCI in treatment group were given intrathecal injection of 3 mg~ kg-1 MPss dissolved in 0. 5 mL sodium chloride. The observation group received simple operation to expose the spinal cord but without damaged. The rats undergone SCI in control group were given intrathecal injection of 0.9% sodium chloride. Immunohistochemical method was used to detect the number of expressed GAP -43 in anterior spinal cord anterior horn cells at different period. The rats were tested hind limbs motion furaction by Basso Beattie Bresnahan (BBB) scores evaluation during survival time. Hematoxylin - eosin ( HE ) staining method was used to detect pathological change of injured spinal cord in each group. Results Intrathecal injection of MPss enhanced the expression of GAP-43 in anterior spinal cord anterior horn cells at different times.BBB scores proved that the function of SCI rat hind limbs movements after intrathecal injection of MPss was better. HE showed that intrathecal injection of MPss could relieve histopathological changes after spinal cord injury. Conclusion l'he results of immunohistochemistry show that intrathecal injection of MPss could repair anterior spinal cord anterior horn cells after spinal cord injury, the mechanism maybe that intrathecal injection of MPss could promote the expression of GAP -43 in neurons.
