狼疮脑病患者脑脊液白细胞介素6、肿瘤坏死因子α水平及其临床意义

摘　　要：目的探讨狼疮脑病患者脑脊液白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平及其在狼疮脑病中的临床意义。方法酶联免疫吸附方法检测15例狼疮脑病患者脑脊液中的IL-6、TNF-α水平,将15例慢性头痛但脑脊液常规检测正常患者作为对照组。收集狼疮脑病患者的临床相关资料,评价2种细胞因子在狼疮脑病中的临床意义。结果狼疮脑病组脑脊液IL-6的水平为(237.73±66.29)pg/m L,显著高于对照组的(4.67±2.50)pg/m L(P<0.01)。TNF-α在狼疮脑病组和对照组水平分别为(39.07±24.48)pg/m L和(4.33±2.94)pg/m L,两者相比有统计学差异(P<0.01)。Pearson相关分析显示,狼疮脑病患者中脑脊液IL-6和TNF-α的水平与患者血沉、血清免疫球蛋白、C反应蛋白、补体水平及系统性红斑狼疮疾病活动度评分等临床指标均无显著相关。结论 IL-6和TNF-α可能成为狼疮脑病治疗的有效靶点,其增高机制值得探索。Objective To evaluate the expression levels of interleukin 6（IL-6）and tumor necrosis factor α（TNF-α）in cerebrospinal fluid（CSF）of patients with lupus encephalopathy（LE）,and explore their clinical significance. Methods CSF were collected from 15 patients with LE and 15 patients with chronic headache. The CSF routine examinations in patients with chronic headache were all normal. The expression levels of IL-6 and TNF-α were measured by enzyme-linked immunosorbent assay. Clinical features and laboratory data were collected from the patients with LE.Results The levels of IL-6（237.73±66.29 pg/mL vs 4.67±2.50 pg/mL）and TNF-α（39.07±24.48 pg/mL vs 4.33±2.94 pg/mL）were significantly different between the patients with LE and those with chronic headache（both P 〈0.01）. No significant correlations were found between the levels of IL-6 or TNF-α and erythrocyte sendimentation rate,serum immunoglobulin levels,serum C-reaction protein level,serum component levels,complete levels as well as SLE disease activity index. Conclusion IL-6 and TNF-α could be the new targets in the treatment of LE. The mechanisms of the increased expression of IL-6 and TNF-α in CSF of patients with LE is worthy to be investigated.
