摘  要:目的探讨生长抑素对改善急性胰腺炎患者炎性因子及瘦素水平的效果。方法选取医院收治的急性胰腺炎患者120例,随机分为对照组和观察组,各60例。两组患者均给予急性胰腺炎的常规治疗方案,对照组患者加用奥曲肽,观察组患者加用生长抑素。结果与对照组相比,观察组治疗总有效率显著升高(P<0.05)。治疗48 h后两组患者的血浆瘦素水平均显著升高,而白细胞介素(IL)-1、肿瘤坏死因子-α(TNF-α)及髓过氧化物酶水平均显著下降(P<0.05),且观察组患者上述指标的变化幅度更显著(P<0.05)。两组患者血浆瘦素水平和IL-1及TNF-α、髓过氧化物酶均呈负相关关系(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义。结论采用首次大剂量静脉注射后持续匀速泵入生长抑素,可有效提高患者血浆瘦素水平,降低体内炎性反应,有利于更快速地控制病情,提高临床疗效。Objective To investigate the effect of somatostatin on inflammation factor in patients with acute pancreatitis ( AP ) factor and the leptin levels. Methods Totally 120 patients with acute pancreatitis were randomly divided into the control group and the observa-tion group,60 cases in each group. The two group were given routine treatment of acute pancreatitis,on the basis of the control group was given octreotide,and the observation group were given somatostatin. Results Compared with the control group,the total effective rate of the observation group increased significantly( P ﹤ 0. 05). The plasma leptin levels after 48 h of treatment were significantly in-creased,and IL-1,TNF-α and myeloperoxidase levels were significantly decreased( P ﹤ 0. 05),and the degree of changes of the above indicators in the observation group were significantly higher( P ﹤ 0. 05). The plasma level of leptin and IL-1,TNF-α and myeloperoxidase were negatively correlated in the two groups( P ﹤ 0. 05). The adverse reactions of the two groups had no significant difference. Conclusion For patients with AP,giving a large dose of intravenous infusion of somatostatin,then continuous intravenous in-fusion somatostatin,can effectively improve the leptin level,reduce the inflammatory reaction,control the disease quickly and improve the clinical efficacy.
