螺旋CT直接胆道造影三维重建在胆道支架置入术的应用

摘　　要：目的:评价螺旋CT直接胆道造影三维重建技术在恶性梗阻性黄疸患者行胆道支架置入术中的应用价值。方法:70例恶性梗阻性黄疸患者分为造影组和对照组,经PTC途径或ERCP途径行胆管外引流后行胆道支架置入,造影组40例术前通过外引流管直接胆管造影后行螺旋CT扫描三维重建,根据图像指导行支架置入,而对照组30例则直接行支架置入术。比较两组支架一次放置成功率及相关并发症发生率。结果:三维重建图像可清晰显示胆管各分支形态及梗阻段,造影组支架一次放置成功率(90%)高于对照组(73.3%),而其并发症发生率(15%)低于对照组(33.3%)。结论:螺旋CT直接胆管造影胆道三维重建术图像质量良好,可为胆道支架置入术提供指导,具有良好的应用价值。Objective： To evaluate the application of direct cholangiography with three - dimensional reconstruction by spiral CT scan in the biliary duct stent placing for the patients of malignant obstructive jaundice. Methods： 70 patients of malignant obstructive jaundice were divided into opacification group and control group, all of them received the operations of biliary duct stent placing after extra - drainage of biliary duct via PTC or ERCP approach. 40 patients in opacification group received the operations of biliary duct stent placing according to the images of direct cholangiography with three - dimensional reconstruction by CT scan, and 30 patients in control group received the operation directly without that, then compared the one - time success rate of the biliary duct stent placing and the relative complication incidence between opacifieation group and control group. Results： The three - dimensional reconstruction image clearly display the branching structure of the biliary ducts and the obstructive segments, the one - time success rate of opaeification group （90%） is higher than that of control group （73. 3% ）, while the complication ineidence in opacification group （ 15% ） is lower than that of control group （33.3%） . Conclusions： The image quality of direct cholangiography with three -dimensional reconstruction by CT scan is satisfactory, and it can be used as a guide in the operation of biliary duct stent placing.
