双歧杆菌三联活菌散对婴幼儿湿疹肠道菌群、免疫功能及细胞因子的影响

摘　　要：目的:通过观察双歧杆菌三联活菌散辅助治疗婴幼儿湿疹的疗效,探讨其对患儿肠道菌群、免疫功能及细胞因子的影响。方法:选取2012年3月至2014年3月我院儿科收治的湿疹患儿74例,随机分为观察组和对照组各37例,均给予常规对症治疗,包括丁酸氢化可的松乳膏外涂、8%硼酸外洗、二盐酸西替利嗪滴剂口服等。观察组加用双歧杆菌三联活菌散(培菲康,每包1 g),2次/日,每次半包,饭后半小时温水冲服。两组疗程均为1个月。观察两组疗效及肠道菌群、T淋巴细胞亚群、细胞因子(IFN-γ、IL-4、IL-10)水平变化。结果:治疗1个月后,观察组总有效率97.3%,高于对照组的81.1%;观察组肠道乳酸杆菌及双歧杆菌水平、Th1/Th2值、IFN-γ/IL-4值、IFN-γ水平高于对照组,IL-4、IL-10水平低于对照组;观察组3个月后复发率18.9%,低于对照组的54.0%。以上指标两组比较差异均有统计学意义(P均<0.05)。结论:双歧杆菌三联活菌散能通过改善湿疹患儿肠道菌群,调节免疫平衡,增强免疫功能,改善临床症状,降低复发率,值得临床推广应用。Objective： To explore the influence of triple viable bifidobacterium on intestinal microflora,immunology and cytokines in infants with eczema. Methods： Seventy four infants with eczema were divided randomly into two groups. All infants were afforded conventional measurement and the treatment group were added the triple viable bifidobacterium. After one month,the clinical effective,the levels of IFN-γ,IL-4,IL-10,T cell subset and intestinal microflora were compared between the two groups. Results： The clinical effective of treatment group（ 97.3%） was higher than the control group（ 81.1%）,the recurrence rate was lower than the control group,the quantity of lactobacillus and bifidobacterium were higher than the control group,the levels of Th1 / Th2 and IFN-γ / IL-4 were higher than the control group,the level of IFN-γ was higher than the control group and the levels of IL-4 and IL-10 were lower than the control group（ P〈0.05）. Conclusion： The triple viable bifidobacterium can improve the intestinal microflora,modulate the immune balance,enhance the immunology and reduce the recurrence rate of infants with eczema.
