硝黄冰茶散外敷辅助对髋关节置换术后患肢肿胀及隐性失血的疗效观察

摘　　要：目的：探讨硝黄冰茶散外敷对髋关节置换术后患肢肿胀及隐性失血量的影响。方法选取2014年08月～2015年04月在我科住院行单侧人工全髋关节置换术治疗的40例股骨头坏死患者。其中，男24例，女16例；年龄26～80岁，平均56．06岁。将治疗患者40例随机分为两组，术前分别测量肢体周径、身高、体重。硝黄冰茶散组20例：术后即开始局部外敷硝黄冰茶散治疗；空白对照组20例：人工全髋关节置换术后常规治疗。术前及术后1、3、7d查血常规，记录红细胞压积。计算围手术期失血量。结果两组患者术后第3天与术后第7天两组患肢髌上周径比较有统计学意义（P ＜0．05）。两组间患者围手术期实际失血量、显性失血量差异无统计学意义（P ＞0．05）；但两组间患者的术后隐性失血量比较却有明显差异， P ＜0．05，有统计学意义。结论硝磺冰茶散可有效减少髋关节置换术后肢体肿胀及隐性失血量。Objective To investigate the effect of external application of the XiaoHuangBingChaSan on the limb swelling and hidden blood loss after total hip arthroplasty（THA） .Methods40 patients with femo‐ral head necrosis who underwent unilateral THA in our department from4014 ,08 to 2015 ,04 were select‐ed .Of which ,24males and 16 females ,aged from 26 to 80 years ,average 56 .06years .These cases were randomly divided into two groups .Measure the body circumference ,height and weight preoperative .Xiao‐HuangBingChaSan group（20 cases）：Local external application of the XiaoHuangBingChaSan starts postop‐eratively .The blank control group（20 cases）：The conventional treatment were applied after T HA .Chec‐king the blood routine preoperatively and first day ,third day ,seventh day after T HA ,record the hemato‐crit and calculate the blood loss of perioperative .Results The two groups of patients of the 3th day and 7th day after operation in two groups of patients limb patellar last week diameter compared with statistical significance （ P〈 0 .05） .Tips between the two groups of patients during operation period of actual blood loss volume ,dominant blood loss difference was not statis‐tically significant （ P 〉 0 .05）;but between the two 〈br〉 groups of patients with postoperative hidden blood loss more has significant difference ,P 〈 0 .05 ,with statistical significance .Conclusion XiaoHuangBingChaSan can effectively reduce limb swelling and hidden blood loss after T HA .
