摘  要:目的 探讨CARE Dose4D调控模式在128层MSCT对冠状动脉个体化扫描技术。方法 选取冠状动脉检查患者随机分为常规组(A组)和对照组(B组),每组100例,体质量指数(BMI)18.5~24.9kg/m2,年龄45~80岁,两组各男46例,女54例。对A组和B组图像质量、噪声、信噪比(SNR)、容积CT剂量指数(CTDIvol)和有效剂量(ED)进行评估。两组冠状动脉扫描的差异对比均进行t检验,P〈0.05,表示两组差异有统计学意义。扫描图像质量由2名高年资诊断医师进行盲审。结果 A组〔BMI(23.45±1.86)kg/m2〕和B组〔BMI(23.27±1.93)kg/m2〕的图像噪声分别为(28.25±5.86)HU、(30.19±6.63)HU,SNR分别为13.34±3.54、15.16±4.82,图像质量评分为(3.65±0.48)分和(3.57±0.49)分,两组图像无显著差异(P〉0.05)。而A组和B组的CTDIvol分别为(25.53.10±4.17)mGy和(13.79±3.48)mGy,ED分别为(6.98±1.14)mSv和(3.81±0.94)mSv,B组的CTDIvol和ED均明显低于A组,两组差异有统计学意义(P〈0.05)。结论应用128层螺旋CT行冠状动脉成像时,通过采用实时动态曝光剂量调节技术(CareDose4D)是旨在保证图像质量的同时减少CCTA检查辐射剂量,均衡控制不同体重指数(BMI)个体射线剂量的目的。Objective To evaluate the individual scanning technique of CARE Dose 4D control mode to coronary artery in 128-detector MSCT. Methods The coronary artery examining patients were divded into the common group(Group A) and the low-dose group (Group B) randomly, with 100 cases per group. BMI ranged from 18.5-24.9 kg/m2 and aged from 45 to 80 with 46 male cases and 54 female cases in each group. The image quailty, the noise, the signal to noise ratio (SNR), the volume CT dose index (CTDIvol) and the effective dose (ED) were evaluated between Group A and B. The difference of the two groups coronary artery scanning were compared by t test, P 〈0.05 and there was significant differ- ence between the two groups. The scanning image quality were blind trial by 2 senior radiologists. Results Group A[BMI (23.45± 1.86) kg/m2) and B[BMI(23. 27±1. 93) kg/m2 ). The image noise in the two groups were(28.25± 5.86) HU, (30.19±6.63)HU and the SNR 13.34±3.54, 15.16±4.82, the score of the image quality (3.65±0.48) and (3.57± 0.49) respectively. There was no significant difference in these options between the two groups ( P 〉0.05). The CTDIvol in Group A and B were (25.53±4.17) mGy and (13.79±3.48) mGy respectively. The ED were (6.98±1. 14) mSv and (3.81±0.94) mSv respectively. The CTDIvol and ED in Group B were significantly lower than those in Group A. There was significant difference between the two groups ( P 〈0,05). Conclusion The CareDose4D can achieve the image quality and reduce the CCTA scanning radiation dose and balance the individual dose based on different BMI at the same time when using 128-detector MSCT.
