摘 要:目的总结探讨CBP(连续性血液净化)治疗老年感染性休克致急性肾衰竭(ARF)的临床效果。方法选择50例感染性休克致急性肾衰竭的老年患者为研究对象,所有患者都接受对症治疗及抗感染治疗、纠正酸碱平衡、营养支持治疗等常规治疗,并同时应用CBP治疗,比较CBP前后患者BUN(血尿素氮)、Cr(肌酐)、CO2CP(CO2结合力)、K+(血钾)、CRP(C反应蛋白)以及存活率情况。结果 CBP治疗24 h后心率、BUN、Cr、K+、CRP三项指标明显下降,CO2CP水平上升至正常,且治疗前后相比差异具有统计学意义(P<0.01)。患者死亡率为36%。结论连续性血液净化有助于改善感染性休克致急性肾衰竭患者的病情,减少并发症的发生,降低死亡率,且宜尽早进行CBP治疗。Objective To summarize the clinical effect of CBP ( continuous blood purification ) in the treatment of acute renal failure ( ARF ) in elderly patients with septic shock.Methods 50 cases of septic shock induced acute renal failure in the elderly patients as the object of study, all the patients accept symptomatic treatment and anti infection therapy, correction of acid -base balance, nutritional support treatment and routine treatment, and were treated by CBP, compared before and after CBP patients bun ( blood urea nitrogen) , Cr( creatinine) , CO2 CP ( CO2 combining power) , K+( potassium) , CRP( C-reactive protein) and survival rate of.Results After CBP treatment, the three indexes of 24 h, Cr, BUN, K+, CRP and were significantly decreased, CO2 CP level increased to normal, and the difference was statistically significant( P<0.01).The mortality rate was 36%.Conclusion Continuous blood purification is helpful to improve the patients with acute renal failure caused by septic shock, reduce the incidence of complications and reduce mortality, and should be treated as soon as possible CBP.
