摘 要:目的:分析H型高血压冠心病患者与非H型高血压冠心病患者的冠状动脉病变稳定性及严重程度的差异,探讨高同型半胱氨酸(Hcy)血症对H型高血压冠心病患者的冠状动脉的影响。方法:纳入高Hcy血症的原发性高血压合并冠心病(简称H型高血压冠心病)患者60例;同时入选同期非高Hcy血症的原发性高血压合并冠心病(简称非H型高血压冠心病)患者60例,采用冠状动脉造影确诊,比较冠状动脉病变不稳定性例数、单支病变、双支病变、3支病变例数及其Hcy水平的差异。结果:在60例H型高血压冠心病患者中,稳定性患者占35%,非H型高血压冠心病患者中,稳定性患者占65%,两组间差异具有统计学意义(χ2=10.8,P<0.05);在60例H型高血压冠心病患者中,单支病变、双支病变和3支病变者分别占35%、42%和23%,非H型高血压冠心病患者中,单支病变、双支病变、3支病变者分别占62%、18%和10%,两组间差异具有统计学意义(χ2=9.1,P<0.05)。H型高血压冠心病组中单支病变、双支病变和3支病变者血浆Hcy水平分别为(13.39±2.34)μmol/L、(17.57±2.77)μmol/L和(22.22±3.08)μmol/L,患者血清Hcy水平与患者冠脉病变支数呈正相关,r和直线回归方程分别为r=0.76,Y=0.14X-0.51。结论:血清Hcy水平与冠状动脉病变的稳定性、复杂及严重性相关,降低高Hcy血症可以预防其冠状动脉病变的发生发展。Objective:To explore the relationship between homocysteine and coronary artery lesions in patient with H type hypertension with coronary heart disease.Method:60cases with H type primary hypertension with coronary heart disease were chosen.Other 60 cases with non H type primary hypertension with coronary heart disease were chosen.These patients were diagnosed by coronary arteriography.We compared the coronary artery lesions instability and the number of single branch lesion,double branch lesions,and three branch lesions.Plasma homocysteine levels were detected by ELISA method.Result:There were 21patients(35%)with stability high blood pressure coronary heart disease,39patients(65%)with instability hypertension coronary heart disease.The21(35%)patients with single branch lesion,25patients(42%)with double branch lesions,14patients(23%)with three branch lesions in H type hypertension coronary heart disease group.The 37patients(62%)with single branch lesion,17patients(18%)with double branch lesions,6patients(10%)with three branch lesions in non H type hypertension coronary heart disease group.In 60 patients with H type hypertension coronary heart disease,Plasma Hcy levels were 13.39±2.34μmol/L in single branch lesion group,17.57±2.77μmol/L in double branch lesions group,2.22±3.08μmol/L in three branch lesions group.There were positive relationship between plasma Hcy levels and coronary lesions count with r=0.76,Y=0.14X-0.51.Conclusion:Homocysteine is involved in the development of coronary artery lesions in H hypertension coronary heart disease patients.
