胃癌患者外周血CD4＋CD25＋调节性T细胞的变化及其意义

摘　　要：目的 应用流式细胞术检测胃癌患者外周血中CD4＋ CD25＋调节性T细胞和血清白细胞介素（IL）-10水平的变化,并探讨其意义.方法 应用免疫荧光流式细胞仪测定65例胃癌患者及35例健康体检者外周血T细胞亚群CD4＋ CD25 ＋/CD4＋比值;酶联免疫吸附试验（ELISA）法检测外周血中IL-10的表达水平.结果 胃癌患者外周血CD4＋ CD25＋/CD4＋和CD4 ＋/CD8＋比值分别是（61.12±5.83）％及（6.67±0.47）％,较健康体检者显著增高,两者差异有统计学意义（P＜0.05）.胃癌患者及健康体检者外周血IL-10的表达水平分别是（43.41 ±0.68） μg/ml及（21.77±0.54） μg/L,胃癌患者较健康体检者显著增高,两者比较差异有统计学意义（P＜0.05）.胃癌患者外周血中CD4＋ CD25＋T细胞水平与胃癌原发肿瘤的侵润深度、淋巴结转移、临床分期及IL-10呈正相关（P＜0.05）.结论 胃癌患者外周血中CD4＋ CD25＋调节性T细胞增多,胃癌患者免疫功能受到抑制.Objective To evaluate the changes of CD4 ＋ CD25 ＋ T cells in peripheral blood of gastric cancer patients,and observe the relationship between CD4 ＋ CD25 ＋ regulatory T cells and the clinicopathologic features.Methods Sixty-five patients with gastric cancer and 35 health examination volunteers were included in this study.The proportion of CD4 ＋ CD25 ＋ T cells population in total T cells was evaluated by flow cytometry.The cytokine production [interleukin （IL）-10] was measured by enzyme linked immunosorbent assay （ELISA）.Results The population of CD4 ＋ CD25 ＋ T cells in peripheral blood from gastric carcinoma patients was （61.12 ± 5.83）％.It is significantly higher than that in health examination volunteers （P ＜0.05）.The population of cytokine production （IL-10） in peripheral blood from gastric carcinoma patients was （43.41 ±0.68） μg/L.It is significantly higher than that in health examination volunteers （P ＜ 0.05）.The CD4 ＋ CD25 ＋ T cells population in gastric cancer patients was positively correlated with the deepness of invasion and IL-10 level （P ＜ 0.05）.Conclusion CD4 ＋ CD25 ＋ regulatory T cell is prevalent in gastric cancer.It may be responsible for immune suppression in gastric cancer patients.
