股深动脉重建治疗下肢动脉硬化闭塞症效果观察

摘　　要：目的探讨股深动脉成形术对改善下肢动脉缺血的疗效。方法选择经下TA（下肢动脉CT造影）检查后发现股浅动脉完全闭塞伴远端流出道条件差的病例,对20例FontaineⅢ、IV期下肢动脉硬化闭塞患者实施股深动脉重建成形。结果 12例Ⅲ期病例术后患肢温度明显升高,疼痛消失或减轻,术后3个月疼痛完全消失。8例IV期患者中7例患肢溃疡于术后3~6周愈合。1例术后患足行足截肢。20例患者术后踝肱指数明显升高（P=0.041）。结论在股浅动脉完全闭塞伴远端流出道差情况下,股深动脉成形术对下肢动脉硬化闭塞症FontaineⅢ、IV期的患者下肢缺血有明显疗效。Objective To investigate the treatment effect of reconstruction of the deep femoral artery angioplasty for lower limb ischemia. Methods According to CTA right lower extremity arterial occlusive arteriosclerosis appear superficial femoral artery occlusion with poor distal outflow tract of the cases, 20 cases of Fontaine Ⅲ, Ⅳ for patients with arteriosclerosis obliterans of lower extremity deep femoral artery reconstruction shape. Results 12 cases of stage Ⅲ patients after limb temperature significantly elevated in all patients, the pain disappeared or reduced, after 3 months of pain completely disappeared. 8 cases of stage IV patients, seven cases of limb ulcers heal within 3-6 weeks after surgery. One patient suffering from foot row foot amputation. 20 cases of postoperative ankle-brachial index was significantly higher （P〈0.05）. Conclusion In the superficial femoral artery occlusion case, a simple line of deep femoral artery angioplasty right lower extremity arterial occlusive disease Fontaine III, IV patients with lower limb ischemia of curative.
