摘 要:对20只大鼠坐骨神经缺损用邻近的带血管蒂筋膜瓣作了管状桥接。不同存活期组织学检查发现,管桥中及神经远侧残端均有再生纤维。逆行追踪和电生理学方法对再生纤维的功能检测证实,手术45天以后确有再生纤维通过管桥到达坐骨神经远端。荧光染料包埋于管桥远侧的神经断端后,相应脊髓节段的灰质前角及脊神经节中均发现有标记细胞。电生理测试再生纤维动作电位阳性;神经传导速度部分恢复。以上结果提示,带血管蒂筋膜瓣(管)有可能作为一种新型的自体非神经桥接物供修复周围神经缺损之用,临床应用尚需进一步探讨。Sciatic nerve defect of 10mm wasbridged with a nearby vascularly pedi-cled fascial flap in 20 rats.The rege-nerating nerve fibres were observedhistologically in the fascial bridge andin the distal sciatic nerve stump po-stoperatively.Retrograde tracing andelectro-physiologic measurenent alsoshowed that the regenerating nerve fibrespassed through the fascial flap bridgeto the distal stump of the sciatic nerveat 45 days after operation.Labelledneurons could be seen in the correspo-nding ventral horns of the spinal cordand in the spinal ganglions when thefluorescent dyes were embedded at thedistal end of the sciatic nerve.Actionpotential could be induced and ne-rve conduction velocity partially re-covered.The results suggest that fascialflap with blood vessels may be usedas a new autogenous non-neural brid-ging substance for the defect of theperipheral nerve.Further research isnecessary for clinical application.
