老年性聋言语识别率与脑皮层弥散张量成像相关性

摘　　要：目的:应用弥散张量成像技术,研究老年性聋言语识别能力与气导平均听阈、下丘、额上回及颞横回功能的相关性。方法:25例老年性聋患者进行纯音听阈测试及单音节词最大言语识别率测试,并用弥散张量成像技术检测下丘、额上回及颞横回各向异性分数值(FA)。比较左右两侧下丘、额上回及颞横回的FA值的差异性,并将气导平均听阈(PTA)、下丘FA值、额上回FA值及颞横回FA值的测试结果与言语识别率测试结果进行双变量相关分析。结果:双侧下丘FA值差异无统计学意义(P>0.05),而左侧额上回及左侧颞横回的FA值明显高于右侧(P<0.05)。言语识别率与PTA呈负相关(P<0.01),与颞横回FA值呈正相关(P<0.05),与额上回FA值呈负相关(P<0.05)。结论:本研究结果说明老年人言语识别率不仅与外周听觉功能有关,而且与中枢听觉和认知功能有关。Objective：To investigate the relationship between pure-tone average （PTA）, the fractional anisot- ropy （FA） of the auditory pathway, cognitive cortex and auditory cortex in presbycusis. Method：Twenty-five eld- erly subjects with presbycusis were participated in the study. PTA, speech discrimination abilities were evaluated in each subject. Diffusion tensor imaging （DTI） was applied to access the FA of the IC, the superior frontal gyrus and the Heschl＇s gyrus. Compare the difference between two sides of the values of FA in the three areas. Bivariate correlation analysis was performed to evaluate the effects of PTA and FA of the inferior colliculus （IC）, the supe- rior frontal gyrus and the Heschl＇ s gyrus on speech discrimination abilities. Result： There were no significant differences between the left and right side of the inferior colliculus （P〉0.05）. Higher FA values were recorded at the left side of the Heschlrs gyrus and the superior frontal gyrus （P〈0.05）. Both PTA and the FA of the superi- or frontal gyrus have a negative association with speech discrimination abilities（P〈0.01, P〈0.05）, while the FA of the Heschl＇s gyrus has a positive association with speech discrimination abilities（P〈0.05）. Conclusion： Our findings indicated that the speech discrimination abilities of the elderly is not only related to the peripheral auditory function, but also to the central auditory and cognitive function.
