经食管二维及三维超声心动图在诊断心脏肿瘤中的应用价值

摘　　要：目的探讨经食管二维及三维超声心动图对心脏肿瘤的诊断价值。方法对经胸超声诊断的23例心脏肿瘤和4例疑似肿瘤病变行经食管二维及实时三维超声心动图检查，观察各肿瘤及其相邻结构的解剖特征和血流动力学变化等情况，对其大小进行估测并与手术结果对照。结果经食管超声较经胸二维超声更能准确分辨心脏肿瘤的位置、毗邻关系以及评价瘤蒂、滋养血管等细小结构情况，经食管实时三维超声能获得比二维超声更加丰富的形态学信息。结论经食管超声心动图较经胸超声心动图在心脏肿瘤的定位、肿瘤的诊断与鉴别方面具有更大的应用价值。Objective To evaluate the diagnostic value of two-dimensional transesophageal echocardiography （2D-TEE） and three-dimensional transesophageal echocardiography （3D-TEE） for cardiac tumor. Methods Twenty-seven subjects were examined by 2D-TEE and 3D-TEE, 23 of them were diagnosed and the other 4 were highly suspected with cardiac tumor through transthoracic echocardiography （TTE）. The anatomical characteristics of the tumor as well as the adjacent structures were carefully observed, meanwhile, the hemodynamic changes were strictly evaluated. The tumor size were compared with operation results. Results Compared with TTE, TEE can locate cardiac tumors more accurately, provide more morphologic information about some tiny structures such as the tumor pedicle, the nourishing vessels and the surrounding tissue to help us fully recognize the tumor. Conclusions TEE has higher application value than 2D-TTE in the location and the differential diagnosis and identification of cardiac tumors.
