摘  要:目的探讨2型糖尿病(T2DM)与维生素D受体(VDR)基因多态性的关系,为临床研究和治疗T2DM提供依据。方法收集2004年6月至2006年1月在天津医科大学总医院住院的天津及其周边地区的233例T2DM为研究对象,均为汉族,无血缘关系,其中男性129例,女性104例,年龄29~76岁,平均(48.0±13.9)岁,病程为1个月~23年,中位病程5.4年,并随机选择同期本院体检的天津地区汉族无血缘关系的236例健康人为对照组,其中男性131例,女性105例,年龄31~75岁,平均(47.8±13.2)岁。采用聚合酶链反应限制性片段长度多态性分析两组VDR Apa I和VDR Taq I基因型和等位基因频率。结果 T2DM患者VDR Taq I基因型和等位基因频率分布与正常组相比,差异无统计学意义(P〉0.05);T2DM患者VDR Apa I基因型aa和等位基因a的频率分别为49%、69%,高于正常对照组(分别为35%、54%),差异有统计学意义(P〈0.01)。结论 VDR Apa I基因型aa和等位基因a可能为天津地区汉族T2DM的易感基因型和易感基因。Objective To explore the association of vitamin D receptor(VDR) Apa I and Taq I genetic polymorphisms with type 2diabetes mellitus(T2DM) in Han residents of Tianjin, and provide the base for clinic investigation and treatment. Methods In this study 233(129 males and 104 females) patients with T2 DM selected from inpatients of our hospital during June of 2004 to January of 2006 were served as subjects. The mean age of patients was(48.0±13.9) years old(26-76 years old). The median of disease duration was 5.4 years(1 month-23 years). And 236 normal residents(131 males and 105 females) served as controls,the mean age of controls was(47.8±13.2) years old(31-75 years old). All subjects and controls were Han residents. The VDRApa I and VDR-Taq I geno-types and allele frequencies were analyzed by polymerase chain reaction fragment length polymorphism(PCR-RFLP) in both groups. Results There were no significant differences of VDR-Taq I genotype and allele frequency between T2 DM patients and controls(P〉0.05). However, the frequencies of Apa I genotype aa and allele a in T2 DM patients were 49% and69%, which were significantly higher than those(35% and 54%) in controls(P〈0.01). Conclusion The VDR-Apa I aa genotype and allele a may be a susceptible gene for the development of T2 DM in Han residents in Tianjin.
