摘　　要：目的探讨能量多普勒三维血管成像（3D-PDA）技术在胎儿脐带绕颈宫内复位干预中的应用价值。方法随机选择孕23～36周有脐带绕颈正常单胎妊娠,分为干预组与非干预组各100例胎儿,根据3D-PDA技术检测脐带绕颈缠绕方向及类型,干预组进行相应的干预措施,指导孕妇通过卧姿促进胎儿翻身使缠绕的脐带回旋复位,比较两组脐带绕颈消失成功率,以及两组不同孕周脐带绕颈复位成功率。结果经过1个干预周期后,干预组的干预成功率为92%,非干预组自然复位率为31%,差异有统计学意义（P〈0.05）。干预组不同孕同脐带绕颈复位成功率显示高于同期比较的非干预组,差异均有统计学意义（均P〈0.05）;且干预组孕23～27周、28～32周脐带绕颈复位成功率明显高于孕33～36周,差异均有统计学意义（均P〈0.05）。结论 3D-PDA技术能有效指导孕妇通过宫内复位干预措施消除脐带绕颈的影响,具有重要的临床意义。Objective To explore the application value of 3D-PDA technology in intrauterine reset intervention of umbilical cord around the neck.Methods 23 ～36 gestational age with umbilical cord around the neck of normal singleton pregnancy were randomly selected and divided into intervention group and non-intervention group,each group containing 100 fetus.Umbilical cord around neck winding direction were detected using 3D-PDA technology,and appropriate interventions were proceeded,which guided the pregnant women to make the winding umbilical cord cyclotron restoration through the prone position to promote fetal turnover.Results After 1 intervention cycle,intervention success rate were 92% in intervention group,and 31%in non-intervention group（P〈0.05）.The success rate in intervention group with different pregnant age were higher than that in non-intervention group with the same pregnant age（P〈0.05）,and the success rate in intervention group with 23～27 weeks,28～32weeks was significantly higher than that with 33 ～36 weeks（P〈0.05）.Conclusion 3D-PDA technology can effectively guide intrauterine reset intervention,which eliminates the influence of the umbilical cord around the neck and has important clinical significance.
