声带息肉与早期声带癌患者嗓音评估对比分析

摘　　要：目的对比分析声带息肉与早期声带癌患者在嗓音评估方面的差异。方法对57例声带息肉和24例早期声带癌患者嗓音评估结果进行统计分析。结果在主观评估中,早期声带癌组患者嗓音自测表分析结果明显高于声带息肉组患者,两组在GRBAS评估中差异无统计学意义。在客观评估中,嗓音分析结果显示,早期声带癌患者最长发声时间(MPT)明显长于声带息肉患者,两组在平均言语基频、基频标准差、基频微扰、振幅微扰、S/Z、声门噪声能量、谐噪比方面差异无统计学意义;电声门图结果显示,声带息肉患者的接触幂和闭合率明显低于早期声带癌患者,而声带息肉患者的开放率明显高于早期声带癌患者。在基频、基频标准差、振幅震颤、接触率、接触率微扰和接触幂微扰方面差异无统计学意义。结论嗓音评估作为一种简便、无创的检查,在区分声带的良恶性疾病方面有一定的临床应用价值。Objective To compare the differences of voice assessment of patients suffering from vocal polyps and early vocal cord cancer. Methods The voice quality of 57 patients with pol- yp of vocal cord and 24 patients with early vocal cord cancer were statistically analyzed. Resuits In the subjective assessment, the result of the self-rated table for the patients with vocal cord polyps was obviously higher than that for the patients with early vocal cord cancer, but the results of the GRBAS assessment of the two groups were not statistically significant. In the objective assessment, the result of the voice acoustic analysis showed that the maximum phonation time （MPT） of the patients with early vocal cord cancer was obviously longer than that of the patients with vocal cord polyp, but the frequency standard deviation （ FOSD ） , results of the fundamental frequency （ El3）, fundamental jitter, shimmer, S/Z ration, normalized noise energy （NNE）, harmonic-to-noise ratio （HNR） of the two groups were not statistically significant. The electrottography （EGG） showed that contact interface （CI） and the closure rate of the pa- tients with vocal cord polyp were lower than that of the patients with early vocal cord cancer, but the aperature rate was higher. And there was no significant difference in contact quotiety （CQ）, contact quotiety perturbation （CQP）, and contact interface perturbation （CIP） between the two groups. Conclusion As an atraumatic and simple method, voice assessment has its value on clinical application of identifying begign and malignant voice diseases.
