术前应用地塞米松联合格拉司琼对腹腔镜胆囊切除患者的术后恢复质量影响

摘　　要：目的:探讨术前应用地塞米松联合格拉司琼对腹腔镜胆囊切除患者的术后恢复质量影响。方法:选择216例腹腔镜胆囊切除术治疗的患者,根据抽签法将患者分为联合组(n=70)、地塞米松组(n=72)和格拉司琼组(n=74)。地塞米松组静脉滴注地塞米松,格拉司琼组静脉注射格拉司琼,联合组则同时给予地塞米松和格拉司琼。比较三组术后恶心、呕吐、有镇痛需求的患者比例。比较术后第1、3天的恢复质量。结果:联合组恶心、呕吐、有镇痛需求的患者比例比较明显低于地塞米松组和格拉司琼组,联合组术后第3天时的身体舒适度、情绪状态、自理能力、疼痛、总分较地塞米松组和格拉司琼组明显提高,差异有统计学意义。结论:术前联合使用地塞米松、格拉司琼既可减少腹腔镜胆囊切除术患者的术后恶心、呕吐症状,还可降低疼痛,减少镇痛药的使用,有效地提高了患者的术后恢复质量。Objective：To investigate quality of recovery after operation of using dexamethasone combined with granisetron for patients with laparoscopic cholecystectomy.Methods：216patients with laparoscopic cholecystectomy were selected,according to lottery,they were divided into combined group（n=70）,dexamethasone group（n=72）and granisetron group（n=74）.Dexamethasone group intravenoused drip dexamethasone,granisetron group intravenoused drip granisetron,while combined group intravenoused drip dexamethasone and granisetron.Rate of patients of nausea,vomiting,analgesic requirement were compared between three groups.Quality of recovery of first day,third days after surgery were compared.Results：Rate of patients of nausea,vomiting,analgesic requirement in combined group were significantlylower than those of dexamethasone group,granisetron group.Degree of physical comfort,emotional status,selfcare ability,pain,score of combined group were significantly higher than those of dexamethasone group and granisetron group after the third day of operation,the differences was statically significant.Conclusion：Before surgery,combination of dexamethasone,granisetron can not only reduce postoperative symptom of nausea,vomiting of patients with laparoscopic cholecystectomy,but also reduce pain,reduce use of analgesics,improve effectively Postoperative recovery quality of patients.
