4种漱口液对白色念珠菌抑制效果的实验研究及临床观察

摘　　要：目的探讨4种漱口液对白色念珠菌的抑菌效果;并观察漱口液对放化疗合并口腔白色念珠菌感染的肿瘤患者的临床疗效。方法实验以白色念珠菌为研究对象,实验组采用复方氯己定含漱液、0.02%醋酸氯己定溶液、0.5%聚维酮碘溶液、0.1%西吡氯铵含漱液,对照组采用生理盐水,观察4种漱口液的抑菌效果。选用抑菌效果最明显的2种漱口液,用于放化疗合并口腔白色念珠菌感染的60例肿瘤患者,并将其分成2组,每组各30例,观察其临床疗效。结果实验组的复方氯己定含漱液和0.5%聚维酮碘溶液的抑菌效果明显高于0.02%醋酸氯己定溶液和0.1%西吡氯铵含漱液,差异有统计学意义(P<0.05);复方氯己定含漱液与0.5%聚维酮碘溶液的抑菌效果比较,差异无统计学意义(P>0.05)。临床观察:0.5%聚维酮碘溶液组患者在疼痛持续时间、平均愈合时间方面,与复方氯己定含漱液组比较,差异有统计学意义(P<0.05)。结论基础实验的数据说明,复方氯己定含漱液与0.5%聚维酮碘溶液和对白色念珠菌的抑菌效果最明显。临床试验的数据说明,0.5%聚维酮碘溶液组患者在疼痛持续时间、平均愈合时间方面优于与复方氯己定含漱液组的患者。Objective To investigate the bacteriostatic effect of four kinds of gargles on Candida albicans, and to observe clinical efficacy of gargles on cancer patients with oral Candida albicans infection undergoing chemoradiotherapy. Methods Four kinds of gargles including compound chlorhexidine mouthwash, 0.02%chlorhexidine acetate solution, 0.5%povidone-iodine solution, and 0.1%cetylpyridinium chloride were applied in experiment group while physiological saline was applied in control group then the antibacterial effect of four gargles was observed. The antibacterial gargles with obvious effect were applied among two groups involving 60 cases of cancer patients with oral Candida albicans infection undergoing chemoradiotherapy. R esults Good antibacterial effect was found in compound chlorhexidine mouthwash and 0.5% povidone -iodine solution when compared with that of 0.02%chlorhexidine acetate solution and 0.1% cetylpyridinium chloride and the difference was statistically significant （ P〈0.05）, but no statistical significance was found in the effect comparison between compound chlorhexidine mouthwash and 0.5% povidone-iodine solution （P〉0.05）. Clinically, 0.5% povidone-iodine solution group presented good effect in terms of duration of pain and average healing time when compared with compound chlorhexidine mouthwash, which indicated statistical significance （ P〈0.05）. Conclusion Compound chlorhexidine mouthwash and 0.5% povidone-iodine solution have good antibacterial effect while in terms of duration of pain and the average healing time, 0.5%povidone-iodine solution performs even better.
