核素显像在乳腺癌前哨淋巴结定位中的临床价值

摘　　要：目的:研究核素显像在乳腺癌前哨淋巴结定位中的临床价值。方法:乳腺癌患者27例,手术前6小时进行核素前哨淋巴结显像,对显像阳性的前哨淋巴结进行体表定位,术中根据体表定位利用γ探测仪找出前哨淋巴结,对检出的前哨淋巴结进行病理检查;患者均行腋窝淋巴结清扫,并对清扫的淋巴结进行病理检查;统计其检出率、准确率、假阴性率、假阳性率。结果:27例中显像阳性24例,检出率88.9%,共发现42枚前哨淋巴结,平均1.75枚;用核素前哨淋巴结显像检出的前哨淋巴结预测腋窝淋巴结转移准确率为95.8%,假阳性率为0%,特异性为100%,假阴性为4.2%。结论:乳腺癌前哨淋巴结显像对指导乳腺癌治疗计划的制定有着重要的临床价值。Objective： To study on the clinical value of radionuclide imaging in the diagnosis of breast cancer sentinel lymph node. Methods： 27 cpatients with breast cancer had radionuclide imaging of sentinel lymph node 6 hours before operation. Body surface positioning was made for imaging positive sentinel lymph node. Sentinel lymph node was identified in according to the surface positioning by intraoperative γ detector. Pathology examination was made to the detected sentinel lymph node; Patients underwent axillary lymph node dissection,and pathological examination of the dissected lymph node was performed; The detection rate,accuracy,false negative rate and false positive rate were counted up. Results： 24 cases of Imaging positive in 27 patients were detected. The detection rate was 88. 9%. 42 lymph nodes were found with an average of 1. 75. The accuracy rate of axillary lymph node metastasis prediction by employing radionuclide sentinel lymph node imaging was 95. 8%. False positive rate was 0%,Specificity was 100%. False negative rate was 4. 2%. Conclusion： Sentinel lymph node imaging in breast cancer has important clinical value to guide the breast cancer treatment.
