螺旋CT多平面重建技术基于淋巴结短长径比值法在胃癌淋巴结转移诊断中的临床应用价值

摘　　要：目的:探讨螺旋CT多平面重建技术基于淋巴结短长径比值法在胃癌淋巴结转移诊断中的应用价值。方法:筛选2013年6月至2014年12月于我院就诊的经病理活检证实为N期胃癌患者52例,作为研究对象。所有患者口服产气粉与水建立胃部低张充气状态,并行多层螺旋CT动态增强扫描,后应用图像后处理软件进行多平面重建(MPR),观察患者淋巴结强化程度、淋巴结长径及短长径比值。结果:不同的淋巴结短长径比值、淋巴结强化程度及淋巴结长径其转移性淋巴结CT检出率存在显著差异,并且以淋巴结短长径比值0.7、淋巴结长径8 mm及淋巴结强化程度80 Hu时作为螺旋CT评价转移性淋巴结的判断标准,Kappa一致性检验显示与术后病理结果比较一致性较强。结论:多层螺旋CT多平面重建技术结合胃淋巴结短长径比值法可提高临床对胃癌淋巴结转移诊断能力,并可通过淋巴结短长径比值的变化进行定量、定性分析胃癌的扩散及病变趋势,可为临床提供可靠、准确的诊治依据,具有临床应用及推广价值。Objective： To investigate the clinical application of the multi planar reconstruction technique of multi-slice spiral CT based on lymph node short-long diameter ratio method in the diagnosis of lymph node metastasis of gastric cancer.Methods： To screening 52 cases of N-period gastric cancer confirmed by pathological biopsy treated in our hospital from June,2013 to December,2014 as the research object. All patients took aerogenic powder and water orally to build the stomach low tension of air condition and had dynamically enhanced multi-slice spiral CT scanning. Then multi planar reconstruction（ MPR） was established with the image post-processing software. Lymph node enhancement degree,lymph node long diameter and short-long diameter ratio were observed in patients. Results： There were significant differences among different short-long diameter ratio,lymph node enhancement degree and the CT detection rate of metastatic lymph nodes. When lymph node length diameter ratio of 0. 7,8 mm lymph node diameter and 80 Hu lymph node enhancement degree were took as spiral CT evaluation criterion of lymph node metastasis,Kappa consistency check showed relatively strong consistency with postoperative pathological results. Conclusion： Multi planar reconstruction technique with multislice spiral CT combined with gastric lymph node short-long diameter ratio method can improve the clinical diagnosis of lymph node metastases in gastric cancer. Quantitative and qualitative analyses of the diffusion and change of gastric cancer were carried out with the short-long diameter ratio changes of lymph node can provide reliable and accurate evidence for the clinical diagnosis and treatment,which has clinical application and promotional value.
